Read this guide for information about participating in CNO’s Quality Assurance Assessment. This information is for all nurses (Registered Nurses [RNs], Registered Practical Nurses [RPNs] and Nurse Practitioners [NPs]) who have been selected for the College of Nurses of Ontario's (CNO's) Quality Assurance (QA) Assessment. QA Assessment process What is QA Assessment? QA Assessment is a component of CNO's QA Program, for which members (RPNs, RNs, NPs) are selected to complete. All nurses selected for a QA Assessment must: Submit a 2022 Learning Plan. Complete a practice activity based on the principles of the Code of Conduct. NPs will also complete an NP Case Example in addition to the Learning Plan and Code of Conduct practice activity. Both the Learning Plan (Section A) and the Code of Conduct Practice Activity (Section B) are contained in the QA Assessment template, located on SharePoint. A CNO-assigned QA Peer Coach will assess your submitted activities and will be available for any coaching support you may need following your submission. How do I start? Review our QA Assessment, templates, guides and examples page. This page contains helpful resources to assist you in completing your QA Assessment. Information about the QA Assessment and how to access the QA activities is also provided in your notification email. Microsoft SharePoint, a secure and confidential browser-based platform, will be used to share and manage documents. Learning Plan Download the QA Assessment template from SharePoint. Section A allows you to write your 2022 Learning Plan. You can also reflect on your practice by reviewing the Practice Reflection questions. How do I complete it? You need to identify two learning goals based on your practice reflection. You may consider asking for input from your colleagues to provide you with a greater awareness of your strengths and to make sure your goals are relevant to your practice. You also must select Code of Conduct principles that align with your goals. For information about how your Learning Plan will be evaluated, read Appendix A at the end of this guide. QA Assessment Template Name: QA QUALITY ASSURANCE Date: Collection of Personal Information: Please review the Privacy Policy on the College of Nurses of Ontario’s (CNO) website (www.cno.org/privacy) to understand how your personal information will be used. Reflect on your practice Refer to CNO’s Practice Reflection questions to help you think about your experiences and identify areas of your practice that need professional development. Watch: How to become a reflective practitioner for guidance on how to reflect. 2. Identify your learning needs What are the most important areas in your nursing practice that need improvement? Consider the following: Talking with a peer(s) or colleague(s) from any practice setting about your learning needs. How will addressing your learning needs help you improve your nursing practice? Code of Conduct Practice Activity Section B of the QA Assessment template allows you to review and reflect on the Code of Conduct principles. The Code of Conduct (the Code) Practice Activity demonstrates how you understand and apply the Code principles to your practice. How do I complete it? Select one principle from the Code that best reflects your role and practice setting for each example you provide. Provide a specific example which demonstrates how the principle you have selected applies to your practice. Do not include any information that can identify a patient, family member or colleague. You must provide two examples, each one connected to a different code principle. For information about how the Code of Conduct Practice Activity will be evaluated, read Appendix B at the end of this guide. Practice Standard Section B: Code of Conduct Practice Activity CNO’s Code of Conduct (Code) is an overarching practice standard that describes the accountabilities nurses are expected to uphold. The purpose of this activity is to consider and explain how you demonstrate the Code’s principles in your practice. Practice Standard Please refer to the QA Assessment Guide for complete instructions, including expectations for your submissions. Code of Conduct Instructions: 1. Review the Code of Conduct principles. 2. Select two principles that best reflect your role and practice setting. Take time to review the specific statements under each principle to best inform your understanding about the principle and its application to practice. 3. For each principle you select, write about a specific time in your own practice where you applied the principle. This can be an interaction, an event, or an experience. Each example should demonstrate at least one of the principle’s statements. Do not include any confidential information in your responses. This includes any information that can identify a patient, family member, or colleague. The six principles of the Code: Principle 1: Nurses respect the dignity of patients and treat them as individuals. Principle 2: Nurses work together to promote patient well-being. Principle 3: Nurses maintain patients’ trust by providing safe and competent care. Principle 4: Nurses work respectfully with colleagues to best meet patients’ needs. Principle 5: Nurses act with integrity to maintain patients’ trust. Principle 6: Nurses maintain public confidence in the nursing profession. Code of Conduct Ensure that you fill out all the required fields of the QA Assessment template, including both Section A (the Learning Plan) and Section B (the Code of Conduct Practice Activity). NP Case Example NPs are required to complete an NP Case Example in addition to the QA Assessment template’s Learning Plan (Section A) and Code of Conduct Practice Activity (Section B). The purpose of the NP Case Example is for you to demonstrate the application of standards as outlined in the Nurse Practitioner practice standard using a case example from your clinical practice. Information about the NP Case Example, including a link to a guide on how to complete it, is included in the QA Assessment, templates, guides and examples page. Submitting QA Activities When is the deadline? The deadline for submission is provided in your notification letter. You can submit your QA Assessment any time before the deadline. What happens after I submit my assessment? A CNO-assigned QA Peer Coach will review your QA Assessment. The QA Peer Coach will then write a report for the Quality Assurance Committee about your submission. The committee will review this report and decide if the assessment was satisfactory or if you need to resubmit your document. What is coaching support? Coaching is a supportive way to guide you through your QA Assessment. It is available for all nurses selected for QA Assessment if they are required to resubmit their activities. Coaching provides an opportunity for you to work individually with the QA Peer Coach, previously known as a Peer Assessor, to help you revise your QA submissions as well as your future practice. For example, a QA Peer Coach may provide feedback on your Code Practice Activity to help strengthen your understanding and application of the principles of the Code to help you meet your QA requirements. QA Peer Coaches are experienced nurses trained in coaching principles. If you have questions, contact QAassessment@cnomail.org or 416-963-3922 (1-800 387-5526 ext. 3922). How will I be informed of my results? A copy of the QA Peer Coach's report will be available in your QA SharePoint folder 14 days prior to when you will be presented to the QA Committee. The report will include your assessment result. If you have questions about the content of the report, you may write a letter to the QA Committee explaining your concerns. Your letter must be sent within 14 days of receiving the report. The committee will consider your written submission. Following the QA Committee meeting, you will receive a Decision Letter which will be available in your QA SharePoint folder. Can I delay my participation in QA Assessment? You may be able to delay your participation in QA Assessment if you have an exceptional circumstance. You have the option to request a one-time extension or a deferral. One-time extension You may request a one-time four-week extension. Please contact the QA Program for more information. Deferral To be considered for a deferral, you must complete and submit a deferral form and provide supporting documentation to the QA Committee. The committee will review your submissions and determine if you can delay your participation. To request a deferral, please contact the QA Program. Who can I contact if I have questions? Appendix A Learning Plan evaluation criteria Your QA Peer Coach is looking for the following criteria in your Learning Plan: You have identified two learning goals relevant to your practice Your goals are linked to at least one principle from the Code of Conduct Your learning goals are within your own legislated scope of practice Your goals are associated with your own individual practice Your learning activities are relevant to the learning goals you identified Your goals do not have the potential to negatively impact public trust, patient care or safety Appendix B Code of Conduct Practice Activity evaluation criteria The QA Peer Coach is looking for the following criteria in your Code of Conduct Practice Activity: You provide two examples for the principles you select Your examples are specific and based on your own practice (not hypothetical) Your examples are in your own words Your examples are an interaction, event or experience Your examples clearly demonstrate the selected principles Your examples do not have the potential to negatively impact public trust, patient care or safety College of Nurses of Ontario Quality Assurance: Assessment Guide 8
