OWNERSHIP AND MANAGEMENT Cameo Beauty Academy is operated at 9714 South Cicero Ave. in Oak Lawn, Illinois 60453. The school is owned by Haroc, Inc an Illinois Corporation. Cameo Beauty Academy is licensed by the Illinois Department of Professional Regulation, Illinois State Board of Cosmetology. Owner and President: Owner and Vice-President: Herman A. Harrison Susan Rock MISSION STATEMENT Cameo Beauty Academy is committed to excellence in cosmetology training. The management and staff are dedicated to the cosmetology profession. Our goals are: To be sincerely interested in you and your progress To strive to provide effective, acceptable, up- ‐to- ‐date training in cosmetology SCHOOL FACILITIES The Cameo Beauty Academy has a Facial Room, Modern Beauty Clinic Facilities, Theory and Practical Classrooms, Dispensary, Extensive Library, Student Lounge, Locker Facilities, Beauty Supply Department and Administrative Offices. The 8400 square foot facility is well lighted, nicely furnished, heated and air conditioned for comfort. Since the school is located on a major thoroughfare, students enjoy easy access to public transportation. There is also a large parking lot in the rear of the school. REGISTRATION Cameo Beauty Academy has an open enrollment policy. Appointments for and admissions interview, to determine if you qualify, can be made by calling the school at (708) 636- ‐4660. INTRODUCTION Cameo Beauty Academy has been training students in the field of cosmetology since 1960. The school has earned a reputation for EXCELLENCE AND QUALITY OF EDUCATION. This is evidenced by its graduates who have achieved success in the many different aspects of the cosmetology profession, including: Salon Stylists/Hair Designers, Salon Owners, Cosmetology Instructors, Performing Platform Artists, School Management Personnel, Technicians, Sales Representatives and other key positions for major beauty product and cosmetic manufacturers. Cameo Beauty Academy is a REDKEN PREMIERE SCHOOL. Cameo graduates are preferred by Redken 5th Avenue Salons (over 30,000) throughout the United States. Each program of instruction qualifies the student to take the Illinois State Board of Cosmetology Licensing Examination. SCHOOL OBJECTIVES It is the objective of Cameo Beauty Academy to provide the training necessary to prepare the student with the skills required to successfully graduate, and secure and retain employment in the field of cosmetology. In order to achieve this objective the school does the following: Maintains a highly skilled and qualified teaching staff. Provides the student with a comprehensive curriculum in the basics of cosmetology and related subjects, with emphasis on current techniques utilized in the profession. Teaches the value of professionalism in cosmetology, including high standards of workmanship and personal conduct. Conducts its business in an ethical and educational atmosphere that is a credit to the cosmetology and beauty industry. ADMISSION REQUIREMENTS The school admits as regular Cosmetology students: High School graduates, Holders of High School Graduation Equivalency Diploma (GED). Applicants for Teacher Training Course must (1) be 18 years old, (2) hold a current Illinois Cosmetology license, (3) have either a high school diploma or a GED certificate. DISCLOSURE OF EDUCATIONAL RECORDS Students and parents of minor students, have the right to inspect, review and challenge information contained in their educational records. Educational records are defined as files, materials and documents which contain information directly related to a student, which are maintained by the institution. Written consent is required before educational records may be disclosed to third parties, with the exception of accrediting commissions or government agencies authorized by law. Any inquiries or complaints regarding the above may be directed to the school management. Note: Copies of student records shall not be provided until all fees and charges specified in their contract are paid in full. GRADING SYSTEM Students are examined regularly in theoretical and practical work. Marking and grading are as follows: A 94- ‐100 Excellent B 87- ‐93 Good C 80- ‐86 Average Bellow 80 Unsatisfactory GRADUATION REQUIREMENTS COSMETOLOGY COURSE: Satisfactory progress in both written and practical work has been maintained at 80% or above. Satisfactory attendance has been maintained at 67% or above All tuition requirements have been met Final school examinations both written and practical have been passed with a score of 80% or above. Student has achieved 1500 clock hours. TEACHER TRAINING COURSE: Satisfactory progress in both written and practical work has been maintained at 80% or above. Satisfactory attendance has been maintained at 67% or above All tuition requirements have been met Satisfactory completion of the Teacher Training Progress Sheet, Teacher’s Workbook, and Notebook. Final school examinations both written and practical have been passed with a score of 80% or above. Student has achieved 1500 clock hours. Upon completion of the required hours, and all other graduation requirements having been met, the school will issue a diploma. STUDENT ADVISING The Cameo Beauty Academy administrative office maintains a listing of all of the available Community Service Organizations for the communities served by the school. COURSE OBJECTIVES COSMETOLOGY COURSE The objective of the Cosmetology course is to develop in the student the practical skills, theoretical knowledge and professional attitudes necessary for success in the Cosmetology profession. The goal of this course is to qualify and equip the student to pass the State of Illinois Licensing Examination. After the graduate has successfully passed their State Board Exam, they are qualified to work as a licensed cosmetologist in the State of Illinois. They may cut hair, perform hair styling, perform all phases of manicuring, apply make- ‐up, and give skin care (within the scope of the license) for profit. The Cosmetology Course is 1500 clock hours in length, which can be completed in not less than eight months of full time attendance. The course is a combination of theoretical and practical instruction. Students are able to develop and practice cosmetology skills, under the supervision of licensed instructors, in actual working conditions in the school’s busy clinic. COSMETOLOGY TEACHER COURSE The objective of the Teacher Training Course is to develop in the licensed cosmetologist the teaching skills, theoretical knowledge and professional attitudes necessary for success in the profession of cosmetology education. The goal of this course is to qualify and equip the student to successfully complete and pass the State of Illinois Licensing Examination. Once they have successfully passed their State Board Exam, they may work in any private cosmetology school in the State of Illinois as a Cosmetology Teacher. The Teacher Training Course is 1000 clock hours in length, which can be completed in as little as six months of full time attendance. Approximately one half of the course time is devoted to post- graduate training in cosmetology, the other half is devoted to technical instruction in the areas of educational psychology, teaching methods (theory), teaching methods (practical), and business methods. COSMETOLOGY COURSE CURRICULUM – 1500 HOURS BASIC TRAINING – 150 hours of classroom instruction in general theory and practical application shall be provided which shall include a minimum of the following subject areas: Tools and their use Shampooing Understanding chemicals and use Types of hair Sanitation Hygiene Skin diseases and conditions Anatomy and physiology Electricity Ethics Nail technology Esthetics PRACTICAL CHEMICAL APPLICATION / HAIR TREATMENTS – 500 hours of instruction, which shall be a combination of classroom instruction and hands on experience, shall be provided in the following subject areas: Chemical safety Permanent waving Hair coloring, tinting and bleaching Hair relaxing Hair and scalp conditioning Shampooing, toning and rinsing HAIR STYLING / HAIR DRESSING – 475 hours of instruction in hair styling which shall be a combination of classroom instruction and hands- ‐on experience, shall be provided in the following subject areas: Cutting Thinning Shaping Trimming Curling Hair treatments Marcelling Application of electrical / mechanical equipment SALON MANAGEMENT, SANITATION AND INTERPERSONAL REALTIONS – 200 hours of classroom instruction shall be provided in the following subject areas: Labor law Worker’s compensation Client relations Bookkeeping Marketing and merchandising Pertinent State and local laws and rules Right to know laws Emergency first aid Business ethics Sanitation Electrical devices Personal grooming and hygiene ESTHETICS – 85 hours of instruction are provided. COSMETOLOGY TEACHER TRAINING CURRICULUM – 1000 HOURS The teacher curriculum includes a minimum of 1000 hours as follows: 500 hours of Post Graduate School Training, that includes all subjects in the basic cosmetology curriculum in section 1175.530 including theory and practice. Presentation of material includes the concepts that are intended to be taught and the skills to be acquired during the various phases of basic education. 20 Hours of Education Psychology, that shall include, but not limited to, topics in educational objectives, student characteristics and development, the learning process and evaluation of learning that relates to teaching. These hours are waived on behalf of Cosmetology Teacher Training students who have completed a course in Education Psychology at an accredited college or university. 20 Hours of Teaching Methods (theory) that shall include, but is not limited to: topics in individual differences in learning, lesson planning and design, lesson delivery, assessment of learning performance, classroom management, student motivation and classroom climate. These hours shall be waived on behalf of Cosmetology Teacher Training students who have completed a course in Teaching Methods – Secondary Level at an accredited college or university. 150 Hours of Application of Teaching Methods that include preparation and organization of subject matter which is presented on a unit by unit basis; and presentation of a subject matter through application of varied methods (lecture, demonstration, testing and assignments). Presentations must provide teaching objective to be accomplished and correlate theoretical with practical application. 50 Hours of Business Methods that include inventory, record- ‐keeping, interviewing, supplies, the Illinois Barber, Cosmetology Esthetics and Nail Technology Act of 1985 and 68 IL. Adm. Code 1175 260 Hours of Student Teaching under the on- ‐site supervision of an Illinois licensed Cosmetology teacher. The student teacher shall present theoretical and practical demonstrations to students in the basic curriculum. The State of Illinois allows individuals who have two (2) years of practical experience to be given credit for having completed the 500 hours of post-graduate training. SCHOOL RULES AND REGULATIONS 1. Gum chewing is highly discouraged. It does not look professional. 2. Cell phones are not to be used unless on break or on lunch. This includes sending or receiving text messages. If your cell phone goes off in class or the clinic, you will be asked to place it in your locker. 3. No smoking is allowed anywhere in the school building. 4. No one is allowed to loiter in the reception area or at the desk. 5. All students must park their cars in the rear of the building or on the street, but not next to the building. 6. No student will be called to the phone. Please do not have personal calls. Only emergency messages or messages from a parent or guardian or spouse will be taken and delivered to you. No other messages will be taken. 7. Students should not remove the locks from the lockers. There will be a 10.00 charge for lost locks. 8. No sitting on table or chair arms. A fee for breakage will be charged to the student. 9. All equipment is to be kept in equipment bag or tower. 10. Students must stay on school premises during breaks. 11. No eating anywhere except in the lunch room. 12. If a student needs to switch from Full time to Part time, or vice versa, they will be allowed to switch one time at no charge. After that it will cost 100.00 each time a switch is done. 13. We reserve the right to suspend or expel any student from the school for: a. Insubordination b. Excessive tardiness (lateness) and/or absences c. Non- ‐payment of tuition d. Gossiping, causing discord, or in any way disrupting classes e. Refusing a client f. Foul language 14. The school nor its management is responsible for personal property not locked up or left over 30 days!!! 15. The wearing of hats is not allowed while attending school. The following infractions will result in immediate expulsion: a. Use, distribution or sale of drugs b. Student under the influence of alcoholic beverages or non-prescription drugs; or student caught drinking during school hours c. Forging teacher’s signatures d. Cheating e. Physical violence f. Stealing We have a zero-tolerance policy when it comes to the above infractions. This is for your protection. These rules are subject to periodic review and at the discretion of the management, may be changed or updated and subsequently posted on the bulletin board. TUITION AND FEES The tuition, fees, equipment and uniform charges, include all required equipment, supplies and materials necessary to complete the specific course of study, within the enrollment period. It does not include personal effects such as shoes or other clothing. ALL TUITION PAYMENTS ARE DUE ON TUESDAY. A complete list of equipment can be found on page 16. ADDITIONAL CHARGES Any student who does not complete a course within the allotted enrollment period will be charged an over- ‐contract fee as specified on Insert page 1, for any additional time required for completion. NON-DISCRIMINATION Cameo Beauty Academy does not discriminate on the basis of age, sex, ethnic origin, race, color or religion in its educational programs, admissions, instruction, graduation policies or in any other way. This practice of non- ‐discrimination also extends to employment by the school. EMPLOYMENT REFERRAL SERVICE Cameo Beauty Academy does not guarantee employment. By law, any school or college is prohibited from guaranteeing employment as an inducement to entering school. A referral service is maintained of employment opportunities for licensed graduates of Cameo Beauty Academy. Job listings can be found outside the student lounge on the job opportunity board. For more information on job placement, please contact the school management. ABSENCES AND MAKE-UP WORK All students are given extra time in their enrollment agreement to allow for absences. Students are given two weeks extra time for every five hundred hours of instruction needed. Any work missed is to be made up. The Director of Education will determine when the appropriate time for the make- ‐up work is to be done. Make- ‐up days are Thursdays and Fridays. If a student is going to be absent, it is their responsibility to notify the school LEAVE OF ABSENCE A Leave of Absence may be granted upon rendering a written request to the school for: extended illnesses of the student; illness, birth or death in the student’s family, or for other reasons deemed legitimate by the school management, and which can be substantiated in writing. Leaves can be for no less than THREE WEEKS and no more than TWO MONTHS (60 days). A student may be granted a MEDICAL LEAVE for up to 6 months (180 days) due to extreme circumstances. TARDINESS All students are encouraged to be in class on time. In the event that a student is tardy, they will only be given credit for the actual time they are signed in school. ALL DAY students are to be signed in no later than 9:05AM. If a student does not sign in by 9:05AM than they cannot sign in until 9:30AM. The LATEST a day student can sign in is 12:00 Noon. All EVENING students are to be signed in no later than 6:05PM. If a student does not sign in by 6:05PM than they cannot sign in until 6:30PM. The LATEST an evening student can sign in is 7:30PM. TRANSFER STUDENTS Cameo Beauty Academy may accept up to 500 transfer hours from other schools. Transfer students may be asked to take an evaluation test prior to enrolling. FEDERAL FINANCIAL AID PROGRAMS Cameo Beauty Academy participates in the following programs of Federal Financial Aid for its students: Federal Pell Grants Federal Direct Loan Program: Federal Direct Stafford Loans (Subsidized and Unsubsidized) Federal Direct PLUS Loans Cameo Beauty Academy 11 Brochure 2014.1

FEDERAL PELL GRANTS The Federal Pell Grant is an award of money to help first time undergraduate students pay for their education. This type of aid DOES NOT HAVE TO BE PAID BACK. Awards for the 2014-15 fiscal years are given based on need, to a Maximum of 5,730.00 for a full Academic Year. Visit www.fafsa.ed.gov to apply. FEDERAL STAFFORD LOANS (SUBSIDIZED AND UNSUBSIDIZED) Federal Direct Stafford Loans are low- ‐interest loans for qualifying students. Loans are made directly by the Federal Government. The maximum amount of Federal Subsidized Stafford Loan per full academic year is 3,500.00. The maximum amount of Federal Unsubsidized Stafford Loan (for independent Students only) is 6,000.00 per full academic year. FEDERAL DIRECT PLUS LOANS The Federal Direct PLUS Loan is for parents who wish to borrow money to help pay for their child’s education. This loan provides additional funds for educational expenses and is available directly from the Federal Government. This loan is approved based on credit worthiness, not on need. The Maximum Amount of PLUS Loan per academic year is found using the formula: Total need in dollars minus other financial aid received. The Federal Direct Loan Website is: www.direct.ed.gov Interest rates and repayment schedules are available upon request. This information may be found on the Federal Direct Loan website, and also be found in the Students Guide to Federal Financial Aid Programs. This pamphlet is published by the United States Department Of Education. The aforementioned booklet as well as all of the applications necessary to apply for Federal Student Financial Aid, are available, free of charge, through the Cameo Beauty Academy Financial Aid Office. These are loans. They must be repaid. REFUND POLICIES The termination date for refund computation purposes is the last date of actual attendance by the students. A. If a student (or in case of a student under legal age, his parent or guardian cancels his enrollment and demands his money back, in writing, within five (5) business days of the signing of an enrollment agreement, he shall be entitled to a refund of all monies paid to the school. The cancellation date will be determined by the post- ‐mark on written notification, or the date said information is delivered to the school administrator or admissions director in person. This policy applies whether or not the student has actually started training. B. REJECTION: An applicant rejected by the school, prior to entering classes, shall be entitled to a refund of all monies paid. C. If a student (or in the case of a student under legal age, his parent or guardian) cancels his enrollment, in writing, after five (5) business days but prior to entering classes, he shall be entitled to a refund of all monies paid to the school less 100.00, which is retained from the registration fee. D. Any student absent without leave for 30 consecutive days or more without notice will be withdrawn and subject to the refund policy as indicated below. E. When notice of cancellation is given after the fifth day following enrollment but before completion of the student’s first day of class attendance, the school may retain no more than the application and registration fee, plus the cost of any books or materials which have been provided by the school and retained by the student. The costs of books for purposes of refunds is the cost of the books charged to the student, not the cost of the books to the school. F. For students who enroll in and begin classes, tuition adjustment shall be made in the following manner: Cameo Beauty Academy 12 Brochure 2014.1

Formal termination shall occur no more than 15 days from the last day of physical attendance or in the case of a leave of absence, the documented date of return. The school shall mail a written acknowledgement of a student’s cancellation or written withdrawal within 15 calendar days of the postmark date of notification. A student shall give notice of cancellation in writing. Unexplained absence from school for more than 30 days shall constitute notice of cancellation. The date of the cancellation shall be the last day of attendance. G. H. I. J. K. L. M. N. If the school is permanently closed and no longer offering instruction, after a student has enrolled (begun training), the student shall be entitled to a pro- ‐rata refund of tuition. If the school closes before a student has begun classes, the student is entitled to a full refund. If a course is cancelled subsequent to a student’s enrollment, the school shall at its option: Provide a refund of all monies paid; or Provide completion of the course. Any monies due the applicant or student shall be refunded within 30 days after cancellation or termination. Any monies due the school must be paid within 30 days. Example of ground for termination are violence, drug or alcohol use, forgery and stealing. In case of illness, or disabling accident, death in the immediate family, or other circumstances beyond the control of the student, the school can make a settlement which is reasonable and fair to both. A student is only allowed to re- ‐enter into the program 1 time, at which time a re- ‐entry fee of 100.00 will be charged Cameo Beauty Academy 13 Brochure 2014.1

FEDERAL POLICY REGARDING RETURN OF TITLE IV FUNDS The law now specifies how the school determines the amount of the Student Financial Aid that you have earned in the event that you withdraw. The law now requires that when you withdraw during a payment period, the amount of Student Financial Aid that was earned up to that point is determined by specific formula. If you received less assistance than the amount you earned, you will be able to receive those additional funds. If you received more assistance than the amount you earned, the excess funds must be returned. The amount of assistance that you have earned is determined on a pro- ‐rata basis. That is, if you completed 30% of the payment period, you earn 30% of the assistance you were originally scheduled to receive.
