LOCATOR Implants are used to restoremasticatory function for the patient and may be suitable for immediateload if sufficient primary stability of the implant is achieved at the timeof placement. The final treatment option may be determined at the timeof surgery as the clinician must consider the quality of supporting boneand initial insertion torque values of the implants. Immediate function isdetermined on a case-by-case basis and at the discretion of the clinician.3

The speed of insertion should not exceed50rpm. Implant insertion torque should not exceed 70Ncm.Follow the D2/D3/D4 drilling sequence prior to following the D1 drilling sequence.This offers the flexibility to adapt the drilling protocol to the patient’s bone quality.For D1 bone density, an optional cortical drill may be used as the final drillingsequence step for the 3.5, 3.9, 4.4, or 4.9mm diameter implant.If strong resistance occurs before the implant reaches its final desired position,rotate the implant counterclockwise and then continue to insert. Repeat until thefinal desired position is obtained. The next drill size up or cortical drill (if available)may also be used if strong resistance occurs before the implant reaches its finaldesired position.NOTERecommended drilling speed is 800-1200RPM. Do not exceed a maximum of800 RPM when utilizing tissue punch.13

PRE-OPERATIVE TREATMENT PLANNINGEvaluate available bone width at desired implantpositions by using the index finger/thumb technique ora ridge mapping instrument (which can be purchasedthrough most dental instrument companies).Measure gingiva height at each planned implantlocation using a periodontal probe to determine theproper LOCATOR Abutment cuff height.3A-3BA panoramic radiograph or CBCT withradiographic markers may be used to evaluatethe bone topography and determine the appropriateimplant positions.AB14

PRE-OPERATIVE TREATMENT PLANNING CONT.Choose the appropriate implant size for thepatient. Zest recommends placement of the LOCATOR Overdenture Implants where patients have at least1mm of available bone around the circumference of theimplant.TIP: Digital implant libraries are available. Formore information, contact a Zest Dental Solutionsrepresentative or visit www.zestdent.com.Determine if the patient’s existing overdenture(s)will be used or if new ones will be fabricated. If anew overdenture is fabricated, follow the standardoverdenture fabrication protocols.Optional: A surgical guide for implant placement maybe fabricated prior to surgery.15

MANDIBULAR IMPLANT PLACEMENTAfter patient selection and evaluation protocols have been completed, determine the number of implantsrequired and discuss all treatment options with the patient. Zest Dental Solutions recommends a minimumof four implants to be placed in the mandible (option of two implants when all standard ridge implants areplaced) and up to six in the maxilla for optimal retention.TIPSIt is recommended to follow the D2/D3/D4 drilling sequence prior to following the D1 drillingsequence. This offers the flexibility to adapt the drilling protocol to the patients bone quality.For D1 bone type, an optional cortical drill may be used as the final drilling sequence step.The cortical drill may be used to a depth of 2-4mm.MANDIBULAR PLACEMENT OF FOUR 2.9MM X 12MM IMPLANTSUsing a surgical guide or by free hand, mark theimplant osteotomy locations using the Starter Drill to drillthrough the gingiva and into the bone crest 6mm. Notethe gingival height. The recommended drilling speed is800-1200rpm.FPO16654321MMMMMMMMMMMMRemove the gingival cores ateach site using the appropriateRotary Tissue Punch (07373,07534). Place the guide pin portionof the Rotary Tissue Punch into thePilot Drill osteotomy and advancethe drill unit to cut away the gingiva.Advance the Rotary Tissue Punch tothe laser depth mark correspondingto the gingival height measurement.The recommended drilling speed isbetween 200-500rpm.

MANDIBULAR IMPLANT PLACEMENT CONT.Place the large diameter end of the DirectionIndicator (07365) into the Starter Drill osteotomies toverify the proper alignment. Attach the proper lenghtDrill Stop onto the Starter Drill according to the desiredrilling depth. The recommended drilling speed is 8001200rpm. Continue osteotomy preparation to the desiredepth at each implant site.TIPS: Alternatively, drill to the proper laser depth markingon the drill calculated by adding the implant lengthplus the gingival tissue height. If divergence is shown to be undesirable basedon the Direction Indicator, use a tool, such as aLindemann drill, to correct.Place the proper length Drill Stop onto the 1.6mmdrill according to the desired drilling depth. Therecommended drilling speed is 800-1200rpm. Continueosteotomy preparation to the desired depth at eachimplant site.Place the 1.6mm diameter (large) end of theDirection Indicator into the osteotomies to verifyproper alignment. Place the proper length Drill Stoponto the 2.1mm drill (07539) according to the desireddrilling depth. Drill 4mm short of the full implantlength. The recommended drilling speed is 8001200rpm. Continue osteotomy preparation to the desireddepth at each implant site.17

MANDIBULAR IMPLANT PLACEMENT (FLAPLESS)Remove the implant package fromthe box and peel back the seal from the plastic tray.Place the sterile implant vial on the sterile surgical tray.The contents of the plastic tray are sterile and shouldonly contact components within the sterile field.B7A-7BRemove the Housing from the implant vialand do not discard. The LOCATOR Abutment isincluded in the Housing. Set the drilling unit speed at30rpm and the placement torque at 35Ncm. Place theImplant Latch Driver (07357) in the handpiece. Seat itonto the hex on the top of the implant and press downto engage securely. The bottom of the driver shouldcontact the abutment seating surface and fully engagethe entire length of the implant hex.AB8A-8BRemove the implant from the vial. Carrythe implant to the mouth, place it into the osteotomyand insert at 30rpm. Use the Latch Driver to drivethe implant three quarters (3/4) of the way intothe osteotomy and finalize insertion with a TorqueIndicating Ratchet Wrench (07362).TIP: Do not use an implant that comes intocontact with any non-sterile area. Replace with a newsterile implant.A18B

MANDIBULAR IMPLANT PLACEMENT CONT.9A-9B Assemble theRatchet Insert and theTorque Indicating RatchetWrench (07362) to finalizeseating. Short and LongImplant Drivers are availablein the surgical kit (07360,07361).BA10 Engage the Implant Driver onto the hex on the topFPOof the implant and verify that it is fully engaged. Slowlyratchet the implant to full depth. If final seating torquemeasures 30Ncm or above, the implant may be placedinto immediate function at the discretion of theclinician, with the patient adhering to recommendedpost-surgical hygiene and care protocols.If the final seating torque measures below 30Ncm,relieve the overdenture acrylic and place a soft liner inthe overdenture around the LOCATOR Abutmentsduring the implant integration period. If 70Ncm oftorque is reached prior to full seating, rotate theimplant counterclockwise and then continue to insert. Repeat until the final desire position is obtained. Thenext drill size up may also be used if strong resistanceoccurs before the implant reached its final desiredposition.NOTEImmediate load protocol is not recommeded unless all implants achieve at least30Ncm of torque. Implant insertion torque should not exceed 70Ncm.19

LOCATOR HEALING ABUTMENT & LOCATORABUTMENT PLACEMENTHEALING ABUTMENT PLACEMENT1Optional: If the implant does not reach a finalseating torque of 30Ncm, a LOCATOR HealingAbutments (07339 or 07340) are available. Use a0.050 inch (1.25mm) Hex Driver and thread the HealingAbutment with the appropriate cuff height on theimplant until finger tight. Relieve the overdentureacrylic and place a soft liner in the overdenture aroundthe LOCATOR Healing Abutments during the implantintegration period.LOCATOR ABUTMENT PLACEMENT2A-2BOpen the flip Housing on the top of thevial Housing and remove the LOCATOR Abutment.Place the Abutment Holder Sleeve onto the LOCATORAbutment Driver and insert into the triangular channelof the LOCATOR Abutment.AB3A-3BAThread the LOCATOR Abutment ontothe implant until finger tight. If the implant placementtorque was 30Ncm or greater, the Abutments maybe tightened to the recommended torque level of30Ncm. If the implant placement torque did not reach30Ncm, the Abutment should only be hand tightened.Assemble the LOCATOR Abutment Ratchet Insert andthe Torque Indicating Ratchet Wrench (07362) withLOCATOR Square Torque Driver (08926) to torque theattachments to 30Ncm.B4If the implant placement torque was less than30Ncm, relieve the overdenture acrylic and place asoft liner in the overdenture around the LOCATORAbutments during the implant integration period.FOR IMMEDIATE LOADING20Proceed with proper precautions: If the implantplacement torque was 30Ncm or greater for allimplants, follow the steps for processing the LOCATORDenture Housing and Inserts into the overdenture.

IMMEDIATE LOADING PROTOCOLSDuring the placement of LOCATOR Overdenture Implants, a clinician may wishto utilize the implants for immediate function. Immediately after placement ofthe implants, the Abutment may be connected to the prosthesis by embeddingthe Housing within the prosthesis. This process results in converting a completedental prosthesis into an implant-retained, tissue-supported prosthesis and willresult in the dental implants in immediate function.While there are many factors involved with successful immediate loading, theoverriding concern is to ensure minimal functional and parafunctional forces arepresent. Minimizing these forces, known as micromovement, is critical to thelong-term health and success of LOCATOR Overdenture Implants. 1-2Un-splinted, free-standing implants with LOCATOR abutments have shown ahigh success rate when used immediately after placement in short-term andlong-term studies.3-4 A prospective clinical study show LOCATOR OverdentureImplant System implants used in immediate function shows a high degree ofsuccess.5Immediate function results in instant gratification and improvement inmasticatory function, enhanced retention of the prosthesis, and improvementin patient quality of life.6 When possible, flapless implant placement can greatlyfurther enhance patient comfort, enhance blood supply to the implant site, and issuccessful long-term.2, 7-9NOTEImmediate loading of implants should be done with caution and proper evaluation asimmediate loading can increase the rate of implant failure.1.
