This documentation must be presented inaddition to the required original Social Security card and thegovernment issued non-expired, photo ID.Military: Healthcare and Medical Occupations:Per the Ohio Administrative code 3701-18-23, an individualwho has the equivalent of twelve months or more of full-timeemployment in the preceding five years in a position thatincludes the provision of direct patient care involving theperformance of daily living activities such as toileting, bathing,feeding, dressing, etc., may submit documentation for approvaland written verification to waive the Nurse Aide Trainingrequirement to sit for the Ohio Nurse Aide exam. Thoseindividuals with military occupational training and experiencein the United States Armed Forces in positions including, butnot limited to, Medical Specialist, Healthcare Specialist, andHospital Corpsman, may qualify for this waiver if they canprovide documentation of at least sixteen hundred (1600)Page 12 V. 16.1 OH STNA Candidate Handbook Effective September 2019

It is your responsibility tocheck for updates before your testing day. Plan on 6 hours for your Nurse Aide TestArrive 20 minutes PRIOR to your SCHEDULED testing timeYou must be dressed in full clinical attire for both writtenand/or skill test (scrub shirts, scrub pants/skirts andclinical footwear. NO CLINICAL TEES WILL BE ACCEPTED!Your original Social Security Card, NO COPIES WILL BEACCEPTED. Name on the Social Security Card and Driverslicense MUST match, if they do not a COPY of the supportingdocumentation must be presented on testing day to the TestObserver. A copy MUST be brought as the Test Observerwill NOT have access to make copies. This is yourresponsibility!US Government issued non-expired, signed, photo id(example: driver’s lic, signed Military Id, Conceal CarryLic, Passport that is non-expired, signed).If you arrive without the required id’s or without full clinicalattire you will be deemed a no show and forfeit your testingfee’s.Also, Per DMV you can use your letter issued from the DMVthat you received until your new Drivers License is mailedto you. Remember, you MUSTbring a copy for return to D&S Diversified. If you do not make the name spelling correction ontesting day you will be charged a fee of 25.00 for areplacement card for the name correction. So, make sure toverify for accuracy on your testing day. It isyour responsibility to bring several sharpened Number 2 pencilswith erasers if you are taking a paper written test. DO NOT BRING orUSE INK PENS. Ink will not allow your scan form to be processed bythe scanner.Prior to your testing day, we strongly recommend that you checkthe web site for any updates prior to your testing day.Your Skill TestYour Skill Test will consist of five scenarios associated with your five(5) assigned skill tasks which will be read to you before you begineach task. Listen carefully to all instructions given by the testobserver. You will have 35 minutes to complete all five skill tasks.A calculator will be available for you if needed, just ask the testobserver.Be sure you understand all instructions before you begin your skilltest. You may not ask questions once the skill test begins.REMEMBER you must actually perform each step to get credit forthat step. For example, you must actually pull the privacy curtain oractually give the resident the call light. Verbalizing the step will notget you credit for that task.The purpose of the skill test is to evaluate your nurse aide skills. Allstudents will be tested on Hand Washing, and four (4) additional skilltasks will be randomly selected from the list for you to perform onyour skill test.The steps that are listed for each skill task are the steps required fora nurse aide to competently perform the task. You will be scored oneach of these steps.You must have a score of 80% on each task without missing any ofthe BOLDED key steps to pass the skill portion of the test. If you fail askill task, you will have to take another skill test with five (5) tasks, atleast one of which will be one that you failed, if you fail one skill. Theknowledge test may be taken orally if you have difficulty readingEnglish. The oral test MUST be requested at the time ofregistration and there is an additional fee. The knowledge test willbe provided along with an audio recording of the questions. Youranswers will be recorded on the scan form just as with theknowledge test. You will hear the WebETest questions on thecomputer.You may request an oral version of the Knowledge test but, thelast 20 questions MUST be read and answered by the studentwithout aide of the recording. These last twenty (20) questionsserve as the reading comprehension requirement required by theOhio Department of Health.
