COMMONWEALTH OF MASSACHUSETTSNOTE: The following requirements reference various Massachusetts and national codes not contained in this document.REQUIREMENTS FOR THE COMMONWEALTH OF MASSACHUSETTSFor all side wall horizontally vented gas fueled equipment installed in every dwelling, building or structure used in whole or in part forresidential purposes, including those owned or operated by the Commonwealth and where the side wall exhaust vent termination is less thanseven (7) feet above finished grade in the area of the venting, including but not limited to decks and porches, the following requirements shallbe satisfied:INSTALLATION OF CARBON MONOXIDE DETECTORSAt the time of installation of the side wall horizontal vented gas fueled equipment, the installing plumber or gas fitter shall observe that a hardwired carbon monoxide detector with an alarm and battery back-up is installed on the floor where the gas equipment is to be installed. Inaddition, the installing plumber or gas fitter shall observe that a battery operated or hard wired carbon monoxide detector with an alarm isinstalled on each additional level of the dwelling, building or structure served by the side wall horizontally vented gas fueled equipment. Itshall be the responsibility of the property owner to secure the services of qualified licensed professionals for the installation of hard wiredcarbon monoxide detectors.In the event that the side wall horizontally vented gas fueled equipment is installed a crawl space or an attic, the hard wired carbon monoxidedetector with alarm and battery back-up may be installed on the next adjacent floor level.In the event that the requirements of the subdivision cannot be met at the time of installation, the owner shall have a period of thirty (30) daysto comply with the above requirements; provided, however, that during said thirty (30) day period, a battery operated carbon monoxidedetector with an alarm shall be installed.APPROVED CARBON MONOXIDE DETECTORSEach carbon monoxide detector as required in accordance with the above provisions shall comply with NFPA 720 and be ANSI/UL 2034listed and IAS certified.SIGNAGEA metal or plastic identification plate shall be permanently mounted to the exterior of the building at a minimum height of eight (8) feet abovegrade directly in line with the exhaust vent terminal for the horizontally vented gas fueled heating appliance or equipment. The sign shallread, in print size no less than (½) inch in size, “GAS VENT DIRECTLY BELOW. KEEP CLEAR OF ALL OBSTRUCTIONS”.INSPECTIONThe state of local gas inspector of the side wall horizontally vented gas fueled equipment shall not approve the installation unless, uponinspection, the inspector observes carbon monoxide detectors and signage installed in accordance with the provisions of 248 CMR 5.08 (2)(a) 1 through 4.EXEMPTIONSThe following equipment is exempt from 248 CMR 5.08 (2) (a) 1 through 4. The equipment listed in Chapter 10 entitled “Equipment Not Required To Be Vented” in the most current edition of NFPA 54 as adoptedby the Board; and Product Approved side wall horizontally vented gas fueled equipment installed in a room or structure separated from the dwelling,building or structure used in whole or in part for residential purposes.MANUFACTURER REQUIREMENTSGAS EQUIPMENT VENTING SYSTEM PROVIDEDWhen the manufacturer of Product Approved side wall horizontally vented gas equipment provides a venting system design or ventingsystem components with the equipment, the instructions provided by the manufacturer for installation of the equipment and the ventingsystem shall include: Detailed instructions for the installation of the venting system design or the venting system components; and A complete parts list for the venting system design or venting system.GAS EQUIPMENT VENTING NOT PROVIDEDWhen the manufacturer of Product Approved side wall horizontally vented gas equipment provides a venting system design or ventingsystem components with the equipment, the instructions provided by the manufacturer for installation of the equipment and the ventingsystem shall include: The referenced “special venting system” instructions shall be included with the appliance or equipment installation instructions; and The “special venting systems” shall be Product Approved by the Board, and the instructions for that system shall include a parts list anddetailed installation instructions.A copy of all installation instructions for all Product Approved side wall horizontally vented gas fueled equipment, all venting instructions, allparts lists for venting instructions, and/or all venting design instructions shall remain with the appliance or equipment at the completion of theinstallation. Installation and repair must be done by a plumber or gas fitter licensed in the Commonwealth of Massachusetts. The flexible gas line connector used shall not exceed 36 inches (92 centimeters) in length. The individual manual shut-off must be a T-handle type.

Vent System:Part #816-CL: For use with minimum 6” x 8” I.D. masonry or 8” I.D. A) PREPARE THE EXISTING FIREPLACEWARNING:CUTTING ANY SHEET-METAL PARTS OTHER THAN COMPONENTS LISTED IN THIS SECTION OF THEEXISTING FIREPLACE IN WHICH THIS GAS FIREPLACE INSERT IS TO BE INSTALLED IS PROHIBITED.1) The refractory, glass doors, screen rails, screen mesh and log grates may be removed from the fireplacebefore installing this gas fireplace insert.2) Any smoke shelves, shields and baffles may be removed if attached by mechanical fasteners.3) If necessary, remove the firebrick on the sides and back of the existing fireplace to obtain at least theminimum opening requirements listed on page #2.4) The fireplace flue damper can be fully blocked open or removed for installation of this gas fireplace insert.5) Remove the existing chimney cap from the chimney.6) Clean the chimney and inside of the fireplace to prevent creosote smell from entering the home.7) Place ‘THIS UNIT HAS BEEN MODIFIED’ label in the bottom of the firebox so it will be visible if this gasfireplace insert is removed.B. RUN THE GAS LINECAUTION:Installation of the gas line must only be done by a qualified person in accordance withlocal building codes.IMPORTANT: DO NOT RUN THE GAS LINE IN A MANNER THAT WOULD OBSTRUCT THEOPERATION OF THE FAN OR FAN COMPONENTS.This fireplace is equipped with a 3/8" flexible gas line connection 18" long.CAUTION:The manual shut off valve and millivolt board flexible gas line must not extend outside the insertunit cavity. See the WARNING label affixed to the flexible gas line for additional installationinstructions and warnings.1. Run the gas line into the fireplace, preferably through the left or right gas line hole provided. A gas lineknock-out is also located in the outer bottom of the insert when running the gas line through the bottom.See Figure B-1 on page #5.NOTE: If installing this insert into minimum opening dimensions, the gas line may need to be run after theinsert is in place due to space limitations.If installing this gas fireplace insert into a factory-built fireplace and the factory-built fireplace has noaccess holes(s) provided, an access hole of 1 ½" or less may be drilled through the lower sides orbottom of the firebox in a proper workmanship like manner. This access hole must be plugged withnon-combustible insulation after the gas supply line has been installed.IMPORTANT: An accessible shut off valve (included with this insert) must be installed up stream from theregulator.Page 4

C. REMOVE THE FOLLOWING ITEMS FROM THE FIREPLACE INSERT:1. GLASS ASSEMBLY:Figure C-1A) Locate the spring-loaded latches securingthe glass assembly (under the firebox).B) While holding onto the glass frame, pull thelatch handles out, then down to release fromthe clips on the glass assembly.C) Lift the glass assembly up off the tabs at thetop of the firebox.D) Pull the bottom of the glass assembly outand remove.E) Set aside where it will not be broken.TWO GLASS LATCHES2. D. MODEL #CSK-31 ONLY: FAN INSTALLATION INSTRUCTIONSThis fireplace insert comes complete with a fan and thermostatic control switch already installed. A speedcontrol, receptacle and 8 ft. fan cord have been installed, wired and mounted in a removable electrical box panelon the right side of the fireplace. If wiring to the pre-installed electrical box is desired, the wiring should be runprior to permanently setting the insert in place and connecting the vent system. A direct wire connection to theelectrical box provided must be performed by a qualified electrician.NOTE: If this fireplace insert is being installed in the minimum opening dimensions, the wiring may need to becompleted after the fireplace insert is set in place.The fan is equipped with a three-prong (grounding) plug attached to the end of the fan electrical cord forprotection against shock hazard and should be plugged directly into a properly grounded three-prong receptacle.Do not remove the grounding prong from the plug. Do not allow any excess fan cord to touch the fireplace.NOTE: Code approved line voltage wiring 14 gauge or better must be used when wiring this assembly. Refer toyour local electrical codes for specific requirements in your area.WARNING:This appliance is equipped with a three-prong (grounding) plug for protection against shockhazard and should be plugged directly into a properly grounded three-prong receptacle. Secure the speed control / receptacle assembly into the electrical box with the (2) screws provided.This appliance, when installed, must be electrically grounded in accordance with local codes, or in the absenceof local codes, with the National Electrical Code, ANSI/NFPA 70, or the Canadian Electrical Codes, CSA C22.1.Page 7

MODEL #CSK-31-RF ONLY: FAN INSTALLATION INSTRUCTIONSINSTALLATION OF THIS FAN WIRING SHOULD BE DONE ONLY BY A QUALIFIED INSTALLER.IMPORTANT: A fan assembly is pre-installed and wired to the RF gas valve in the CSK31-RF models.A receptacle and 8 ft. fan cord have been installed, wired and mounted in a removable electrical box panel onthe right side of the fireplace. If direct wiring to the pre-installed electrical box is desired, the wiring should becompleted prior to permanently setting the insert in place and connecting the vent system. Code approved linevoltage wiring 14 gauge or better must be used when wiring this assembly. Refer to your local electrical codesfor specific requirements in your area. A direct wire connection to the electrical box provided must be performedby a qualified electrician.NOTE: If this fireplace insert is being installed in the minimum opening dimensions, the wiring may need to becompleted after the fireplace insert is set in place.The fan is equipped with a three-prong (grounding) plug attached to the end of the fan electrical cord forprotection against shock hazard and should be plugged directly into a properly grounded three-prong receptacle.Do not remove the grounding prong from the plug. E. INSTALLING THE INSERTIMPORTANT: ALL STEPS IN SECTION ‘A) PREPARE THE EXISTING FIREPLACE’ MUST BE COMPLETED BEFORECONTINUING WITH THIS INSTALLATION.1. INSTALL THE VENT SYSTEMCOMBUSTION AIR VENTING OPTIONS:The combustion air intake pipe may be run the complete length of the chimney and connected to the terminationcap (full connection method), or may be ‘stubbed’ into the existing chimney from inside the fireplace using aminimum of 4' combustion air intake pipe.KOZY HEAT #816-CL CO-LINEAR VENT SYSTEM:The co-linear pipes included in this vent system is designed to extend up to 30 ft.IMPORTANT:The 3" exhaust pipe can be identified by the red marking. It is imperative for properoperation of this insert that the exhaust and combustion air pipes be connected to thecorrect collar both on the termination kit and air duct on the insert.A.
