Majoris Health Systems, Inc.SAIF Corporation, has contracted with MajorisHealth Systems, Inc., a state-certified managedcare organization (MCO) to provide services toemployees injured on the job.We understand that dealing with an injury cansometimes be stressful and confusing. Wetherefore recommend that you become familiarwith the Majoris procedures before you may needto seek care for a work-related injury or illness.Following are answers to some of the morecommonly-asked questions concerning ourmanaged care program. Should you haveadditional questions, please feel free to contactSAIF Corporation at 800.285.8525, or MajorisHealth Systems directly at 800.525.0394.What is Majoris Health Systems?Majoris is a state-certified managed careorganization that contracts with physicians,hospitals, and other health care providers toprovide medical services to covered employeeswith work-related injuries or illnesses. Theseproviders are carefully selected and trained in thetreatment of work-related conditions. Majoris,and the health care providers want to make surethat timely, effective, and convenient medicalservices are available for our covered workers.Majoris will also be working closely with yourdoctor and SAIF Corporation to help you return togainful employment as soon as possible after anon-the-job injury or illness.Is Majoris a workers’ compensationinsurance company?No. Majoris contracts with SAIF Corporation, aworkers’ compensation insurance company.Majoris monitors medical care that is provided toensure that it is appropriate and necessary, andit meets specific quality standards.Majoris does not make decisions on acceptanceor denial of claims, payment of time loss ormedical benefits, or any other workers’compensation benefits. Decisions concerningthese and all other claims issues remain theresponsibility of SAIF Corporation.R2031 Majoris Health Systems, Inc. DirectoryAm I required to see one of the Majoris doctorsif I am injured on the job and need medicalcare?In most cases, yes. Once your claim is “enrolled”with Majoris Health Systems, (which means youhave been given written notice of yourrequirement to treat within the MCO) you will berequired to treat with an MCO provider unlessone of the circumstances explained belowapplies.The situations in which you may receivecompensable care from a non-Majoris providerafter your claim is enrolled are as follows:1. You have a private physician,chiropractic physician, or nursepractitioner who qualifies as a primarycare physician, chiropractic physicianor authorized nurse practitioner.Your family physician, chiropractic physician,or authorized nurse practitioner may qualify totreat you under the managed carearrangement, even if s/he is not on the Majorislist of contracted providers. To qualify: Your provider must be a medical doctor(M.D.) or osteopath (D.O.), chiropracticphysician (D.C.) or authorized nursepractitioner. Your doctor must be a family practitioner,general practitioner, internal medicinespecialist, chiropractor, or authorizednurse practitioner. You must have a history of being treatedby that doctor or authorized nursepractitioner, or have the doctor orauthorized nurse practitioner as adesignated primary care provider underyour group health plan. The doctor or authorized nurse practitionermust agree to abide by all terms andconditions of Majoris Health Systems, andmust refer you to a Majoris provider forany additional care you may need.If your authorized nurse practitioner is qualifiedto provide your care, s/he will be allowed toprovide medical treatment and authorize timeloss for 180 days from the date of the first nursepractitioner visit on the initial claim.Page 2May 2021

There are fewer than three MCOproviders available in a given categoryin Majoris’ geographical service area.You may be allowed to seek treatment from anon-Majoris provider if there are fewer thanthree Majoris providers in the followingcategories: Acupuncturist (LAC)Optometrist (OD)Chiropractor (DC)Dentist (DMD or DDS)Naturopath (ND)Osteopath ((DO)Physician (MD)Podiatrist (DPM)Physical therapist (PT)PsychologistAuthorized nurse practitioner (ANP)All out-of-panel treatment will be subject toMajoris’ utilization and treatment standards.3. You reside outside Majoris’geographical service area.If you reside outside Majoris’ geographicalservice area you may select a non-MCOprovider if they practice closer to yourresidence than an MCO provider of the samecategory and if they agree to the terms andconditions of the MCO.If you think you qualify for any of the aboveexceptions and would like consideration forout-of-panel treatment, please contact youradjuster to begin the review process.Should you receive care from a provider whodoes not meet the above criteria for out-ofpanel treatment, SAIF Corporation will not berequired to pay for medical services.What if I live a long distance from Majoris’service area?If you live more than one hundred miles fromMajoris’ geographic service area, you will not besubject to the MCO arrangement.What if I am currently receiving care from anon-MCO provider for a work-related injuryor illness at the time I am enrolled into theMCO program?You will be required to begin treating with anMCO provider, with the exceptions noted above.Howevery, if you are not yet medically stationaryand think that a change of physicians would bemedically detrimental to you, you may requestreview of your situation by the MCO. To requestR2031 Majoris Health Systems, Inc. Directoryreview, please submit your request in writing tothe address listed within 30 days of the date ofthe action. Failure to request review in writingwithin 30 days precludes further appeal.What if I need help finding or schedulingan appointment with an MCO provider?If you would like help in scheduling anappointment, or if you have contacted severalpanel providers and were not able to schedule atimely appointment, please contact Majoris forassistance at 800.525.0394.What do I do if there is a medical emergencyand I’m not able to see a Majoris provider?In true emergency cases, Majoris, your employerand SAIF Corporation believe the first priority isto take care of the medical emergency at anappropriate medical facility. An emergency isdefined as a medical condition that if treatment isnot rendered immediately, creates the risk ofdeath, serious disability or serious medicalconsequences.If your claim is subject to the MCO, and you arefar away from or otherwise unable to receive carefrom a Majoris provider in an emergency, youshould seek care from the nearest appropriatemedical facility. After you are out of immediatedanger, all follow-up care must be providedwithin the MCO. If emergency care is needed andan appropriate Majoris facility is available, careshould be sought from the Majoris memberfacility if possible.If you are in need of emergency care and unsureof where to go, seek medical care from theclosest available medical facility. However,emergency care should not be used as asubstitute for routine, ongoing medical care froman attending physician.What can I do if I disagree with an actiontaken by Majoris or its member physicians?SAIF Corporation and Majoris both want to makesure you receive timely, effective and convenientmedical services for your work-related injury.However, should a dispute arise concerning yourcare within the MCO, you may request reviewthrough Majoris’ Internal Dispute ResolutionProcess.Such request must be made in writing within 30days of the action giving rise to the dispute. Youshould state the specific issue for which you arerequesting review, and include any informationyou think we should consider in our review.Address your requests to “Administrator,Majoris Health Systems, P.O. timely, we will review the disputed decision andnotify you of our decision within 60 days of yourrequest. Thereafter, if you continue to disagreewith our decision, you may appeal to the Directorof the Department of Consumer and BusinessServices for further review.Please note that failure to submit a request forreview in writing to Majoris Health Systemswithin 30 days of the action giving rise to thedispute means you lose your right of furtherappeal to the Director of DCBS.Remember:Injured workers should remember to tell theMajoris MCO doctor that they were injured on thejob (when and how) and the employer’s name.They will be asked to sign a form (Form 827) sothe doctor can release information concerning theinjury to SAIF. Injured workers and employersshould also complete the Employer’s Report ofInjury (Form 801).
