4.130-1§4.130—Scheduling of ratings—mental disorders4.130-1§4.130 Schedule of ratings²mental disorders.The nomenclature employed in this portion of the rating schedule is based upon theDiagnostic and Statistical Manual of Mental Disorders, Fourth Edition, of the AmericanPsychiatric Association (DSM-IV). Rating agencies must be thoroughly familiar with this manualto properly implement the directives in §4.125 through §4.129 and to apply the general ratingformula for mental disorders in §4.130. 4.130-3§4.130—Scheduling of ratings—mental disorders4.130-3CHRONIC ADJUSTMENT DISORDER9440 Chronic adjustment disorderGeneral Rating Formula for Mental Disorders:Total occupational and social impairment, due to such symptoms as:gross impairment in thought processes or communication;persistent delusions or hallucinations; grossly inappropriatebehavior; persistent danger of hurting self or others; intermittentinability to perform activities of daily living (including maintenanceof minimal personal hygiene); disorientation to time or place; memoryloss for names of close relatives, own occupation, or own name .100Occupational and social impairment, with deficiencies in most areas,such as work, school, family relations, judgment, thinking, or mood,due to such symptoms as: suicidal ideation; obsessional ritualswhich interfere with routine activities; speech intermittently illogical,obscure, or irrelevant; near-continuous panic or depression affectingthe ability to function independently, appropriately and effectively;impaired impulse control (such as unprovoked irritability with periodsof violence); spatial disorientation; neglect of personal appearance andhygiene; difficulty in adapting to stressful circumstances (includingwork or a worklike setting); inability to establish and maintaineffective relationships .70Occupational and social impairment with reduced reliability andproductivity due to such symptoms as: flattened affect; circumstantial,circumlocutory, or stereotyped speech; panic attacks more than oncea week; difficulty in understanding complex commands; impairmentof short- and long-term memory (e.g., retention of only highly learnedmaterial, forgetting to complete tasks); impaired judgment; impairedabstract thinking; disturbances of motivation and mood; difficulty inestablishing and maintaining effective work and social relationships .50Occupational and social impairment with occasional decrease in workefficiency and intermittent periods of inability to perform occupationaltasks (although generally functioning satisfactorily, with routinebehavior, self-care, and conversation normal), due to such symptomsas: depressed mood, anxiety, suspiciousness, panic attacks (weekly orless often), chronic sleep impairment, mild memory loss (such asforgetting names, directions, recent events) .30Occupational and social impairment due to mild or transient symptomswhich decrease work efficiency and ability to perform occupationaltasks only during periods of significant stress, or; symptoms controlledby continuous medication .10(No. 48 5/5/09)

4.130-4§4.130—Scheduling of ratings—mental disorders4.130-4A mental condition has been formally diagnosed, but symptoms are notsevere enough either to interfere with occupational and socialfunctioning or to require continuous medication .0EATING DISORDERS9520 Anorexia nervosa9521 Bulimia nervosaRating Formula for Eating Disorders:Self-induced weight loss to less than 80 percent of expected minimumweight, with incapacitating episodes of at least six weeks total durationper year, and requiring hospitalization more than twice a year forparenteral nutrition or tube feeding .100Self-induced weight loss to less than 85 percent of expected minimumweight with incapacitating episodes of six or more weeks totalduration per year .60Self-induced weight loss to less than 85 percent of expected minimumweight with incapacitating episodes of more than two but less thansix weeks total duration per year .30Binge eating followed by self-induced vomiting or other measures toprevent weight gain, or resistance to weight gain even when belowexpected minimum weight, with diagnosis of an eating disorder andincapacitating episodes of up to two weeks total duration per year .10Binge eating followed by self-induced vomiting or other measures toprevent weight gain, or resistance to weight gain even when belowexpected minimum weight, with diagnosis of an eating disorderbut without incapacitating episodes .0Note: An incapacitating episode is a period during which bed rest and treatment by aphysician are required.
