We can enrich the lives of many families ofchildren and youth with disabilities by using this guide and otherresources available in our community.Uniting Parents is a program funded through a grant fromthe Department of State Health Services. Our sponsoring agencyis the Coalition of Health Services, Inc. Uniting Parents utilizesa parent case management model where the case managers areor have been the parents of a child with disabilities and/or achronic health condition. The case managers are trained and arepassionate in providing a wealth of information to families and tothe community. The parent case managers utilize their experiencesand knowledge to help guide families with their own children withdisabilities through an array of services that can be unfamiliarand sometimes frustrating. Families and parent case managersbecome partners through our case management process andfamilies have access to the following services: education, training,social networking events, advocacy, and respite.There are many programs in our area and they change namesand numbers over time.
