Consultation with Parent/Carers with Children/Young People with SEND:Annual Health Checks for 14 Year Olds and Over with a LearningDisabilityIntroduction & PurposeThis survey was designed to elicit the views of parent/carers acrossDerbyshire in relation to the annual health check (AHC) of young people aged14 year old and over who have a learning disability. This is also sometimesreferred to as the ‘Learning Disability Health Check’.Derbyshire Parent Carer Voice is aware that there is a relatively low take-upacross the county of young people accessing this free annual health check. The survey was designed to find out if parent carers were aware of the annualhealth check, whether their young people have accessed the annual healthcheck and their feedback, if appropriate.Derbyshire Parent Carer Voice will share the results of this survey withcommissioning managers from Derby and Derbyshire CCG as part of theCCG’s plan (targeted at increasing the uptake of the annual health check) aswell as with Healthwatch Derbyshire and other relevant agencies. This short survey opened 31 December 2020 and closed on 15 January 2021.The survey was carried out through Survey Monkey to obtain the views ofparents/carers only.Page 2
