Section 1: Instructions to Offerors1. Introduction: The Global Health Supply Chain – Procurement and Supply Management (GHSCPSM) project is a USAID program implemented by Chemonics International in Liberia. The goal ofthe GHSC-PSM project is to ensure uninterrupted supplies of health commodities in support ofUSG-funded public health initiatives around the world. As part of project activities, the GHSCPSM requires the purchase of internet services for its offices and staff to enable theimplementation of the project. The purpose of this RFQ is to solicit quotations for these services.Offerors are responsible for ensuring that their offers are received by Chemonics in accordancewith the instructions, terms, and conditions described in this RFQ. Failure to adhere withinstructions described in this RFQ may lead to disqualification of an offer from consideration.2. Offer Deadline and Protocol: Offers must be received no later than 4pm local Monrovia time onJanuary 28th,2021 by email. Emailed offers must be emailed to PSMLiberiaRFQ@ghsc-psm.org.Please reference the RFQ number in any response to this RFQ. Offers received after the specifiedtime and date will be considered late and will be considered only at the discretion of Chemonics.3. Questions: Questions regarding the technical or administrative requirements of this RFQ may besubmitted no later than 5pm local Monrovia time on January 26,2021 by email toPSMLiberiaRFQ@ghsc-psm.org . Questions must be submitted in writing; phone calls will not beaccepted. Questions and requests for clarification—and the responses thereto—that Chemonicsbelieves may be of interest to other offerors will be circulated to all RFQ recipients who haveindicated an interest in bidding.Only the written answers issued by Chemonics will be considered official and carry weight in theRFQ process and subsequent evaluation. Any verbal information received from employees ofChemonics or any other entity should not be considered as an official response to any questionsregarding this RFQ.4. Facility Assessment: Interested offerors are highly encouraged to carry out an assessment of theGHSC-PSM offices to determine fiber cabling routes between January 25th to 27th,2021 at 4pm.All cost associated with this assessment is at the sole cost of the offeror. To exercise extremecaution and avoid any structural or property damage, representatives of GHSC-PSM willaccompany the offeror during the walk through: all damage(s) must be reported immediately,and the contractor will be responsible for the cost related to the repair of any reporteddamage(s) resulting from Offeror's failure to exercise reasonable care during the visit. Due toCOVID 19, there are protocols that must be followed before having access to the building. Inorder to obtain access, appointments can be made by contacting Mr. Rennie Williams via emailat rwilliams@ghsc-psm.org with a copy to PSMLiberiaRFQ@ghsc-psm.org5. Scope of Work: Section 3 contains the Scope of Work for the required services.6. Quotations: Quotations in response to this RFQ must be priced on a fixed-price, all-inclusivebasis, including delivery and all other costs. Pricing must be presented in US dollars. Offers mustremain valid for not less than thirty (60) calendar days after the offer deadline. In addition, offerors responding to this RFQ are requested to submit the following: Copy of their official registration or business license. Contact information for at least three (3) references of past or present clients indicatingthe relevant services carried out in the last three (3) years that best illustratecompany/individual’s qualifications and past performance;Cover letter, signed by an authorized representative of the offeror;Company’s profile introduction (max 1 page) to company/individual’s areas of expertise,and description of the team and main clients;The Technical Acceptability Verification Checklist Copy of offeror’s registration orbusiness license;Official quotation7. Delivery: As part of its response to this RFQ, each offeror is expected to provide an estimate (incalendar days) of the delivery timeframe (after receipt of order). The delivery estimatepresented in an offer in response to this RFQ must be upheld in the performance of any resultingcontract8. Source/Nationality/Manufacture: All services offered in response to this RFQ or supplied underany resulting award must meet USAID Geographic Code 935 in accordance with the UnitedStates Code of Federal Regulations (CFR), 22 CFR §228. The cooperating country for this RFQ isLiberia.Offerors may not offer or supply services or any commodities that are manufactured orassembled in, shipped from, transported through, or otherwise involving any of the followingcountries: Cuba, Iran, North Korea, Syria.9. Taxes and VAT: The agreement under which this procurement is financed does not permit thefinancing of any taxes, VAT, tariffs, duties, or other levies imposed by any laws in effect in theCooperating Country. No such Cooperating Country taxes, VAT, charges, tariffs, duties or levieswill be paid under an order resulting from this RFQ.10. DUNS Number: Companies or organizations, whether for-profit or non-profit, shall be requestedto provide a Data Universal Numbering System (DUNS) number if selected to receive an award inresponse to this RFQ valued greater than or equal to USD 30,000 (or equivalent in othercurrency). If the Offeror does not have a DUNS number and is unable to obtain one before thesubmission deadline, Offeror shall include a statement noting their intention to register for aDUNS number should it be selected as the successful offeror or explaining why registration for aDUNS number is not possible. Eligibility: By submitting an offer in response to this RFQ, the offeror certifies that it and itsprincipal officers are not debarred, suspended, or otherwise considered ineligible for an awardby the U.S. Government. Chemonics will not award a contract to any firm that is debarred,suspended, or considered to be ineligible by the U.S. Government.12. from consideration. Chemonics reserves the right to waive immaterial deficiencies at itsdiscretion. the best value based on a lowest-price, technically acceptable basis.Best-offer quotations are requested. It is anticipated that award will be made solely on the basisof these original quotations. However, Chemonics reserves the right to conduct any of thefollowing: Chemonics may conduct negotiations with and/or request clarifications from any offerorprior to award.While preference will be given to offerors who can address the full technicalrequirements of this RFQ, Chemonics may issue a partial award or split the award amongvarious Vendors, if in the best interest of the GHSC-PSM Project in Liberia.Chemonics may cancel this RFQ at any time.Chemonics may reject any and all offers, if such action is considered to be in the bestinterest of Chemonics.Please note that in submitting a response to this RFQ, the offeror understands that USAID is nota party to this solicitation and the offeror agrees that any protest hereunder must bepresented—in writing with full explanations—to the GHSC-PSM project in Liberia forconsideration, as USAID will not consider protests regarding procurements carried out byimplementing partners. Chemonics, at its sole discretion, will make a final decision on theprotest for this procurement.13. Terms and Conditions: This is a Request for Quotations only. Issuance of this RFQ does not inany way obligate Chemonics, the GHSC-PSM project, or USAID to make an award or pay for costsincurred by potential offerors in the preparation and submission of an offer.This solicitation is subject to Chemonics’ standard terms and conditions. Any resultant award willbe governed by these terms and conditions; a copy of the full terms and conditions is availableupon request. Please note the following terms and conditions will apply:(a) Chemonics’ standard payment terms are net 30 days after receipt and acceptance of anycommodities or deliverables. Payment will only be issued to the entity submitting the offerin response to this RFQ and identified in the resulting award; payment will not be issued to athird party.(b) Any award resulting from this RFQ will be firm fixed price, in the form of a serviceagreement.(c) No services or commodities may be supplied that are manufactured or assembled in,shipped from, transported through, or otherwise involving any of the following countries:Cuba, Iran, North Korea, Syria.(d) Any international air or ocean transportation or shipping carried out under any awardresulting from this RFQ must take place on U.S.-flag carriers/vessels.(e) United States law prohibits transactions with, and the provision of resources and support to,individuals and organizations associated with terrorism. The Vendor under any awardresulting from this RFQ must ensure compliance with these laws.(f) The title to any goods supplied under any award resulting from this RFQ shall pass toChemonics following delivery and acceptance of the goods by Chemonics. Section 2: Offer ChecklistTo assist offerors in preparation of the quote, the following checklist summarizes the documentationto include an offer in response to this RFQ: Cover letter, signed by an authorized representative of the offeror (see Section 5 for template) Copy of offeror’s registration or business license (see Section 1.6 for more details); Company’s profile or introduction (max. 1pg) to company/individual’s areas of expertise, anddescription of the team and main clients (see Section 1.5 for more details); The Technical Acceptability Verification Checklist Copy of offeror’s registration or businesslicense (see Section 4 for template) Official quotation, as a separate document, (see Section 3 for scope of work,); Contact information for at least three (3) references of past or present clients indicating therelevant services carried out in the last three (3) years that best illustrate company/individual’squalifications and past performance (see Section 1.5 for more details);PSML RFQ 2021-01-25Page 5 of 10GlobalQMS ID: 10844.1, 5 June 2019

Section 3: Scope of Work and Technical QualificationsThe purpose of this RFQ is to engage the firm to provide internet services to Chemonics and theGHSC-PSM project. The firm shall, and when requested, provide the following services:1. Dedicated high speed 16Mbps as standard internet speed (16mg/16mg) upload and download by means ofdirect fiber optic cable connection.2. Internet connection at a traffic-independent flat fee, unlimited usage time without any further restriction on(i.e. proxy, firewall, filtering servers) and the internet bandwidth must be dedicated line, not share withother customers.3. Installation of the fiber optic cabling (using conduits where necessary) that connects ISPnetwork to office LAN and observe safety measures (grounding is preferred).4. Equipment/media used for the service and ensure proper maintenance and routine checkup is done asnecessary. All equipment shall remain property of the provider and will be on loan/rent to the GHSC-PSMproject during the period of performance.5. Bandwidth monitoring.6. Three Public IP addresses for network management device use.7. Ongoing support to ensure uninterrupted supply of services.8. Written reports on service interruption due to ISP’s equipment or causes within its control.Technical Qualifications that the selected offeror must possess:1. Around the clock, 24/7 helpdesk support and technical assistance (remote or onsite as the need may be)during service downtime to ensure service is restored immediately.2. Qualified technical representatives who may within 2 hours of internet disruption to resolveany problem at no additional cost to GHSC-PSM, if the problem cannot be resolved through telephonesupport or assistance.3. Bandwidth monitoring tools which allows the monitoring of bandwidth and visualize network usage at afine-grained level and optimization solution through a graphical portal.4. Backup/redundant internet plan/link to ensure uninterruptable internet service provision(with 99.9% uptime availability)5. Guarantee redundant routes with appropriate internet speed corresponding to the main link i.e. multipeering routes or any alternative routes that will provide access to the internet in case of problems withinternet Services Provider’s primary connections to ensure 99.9% uptime service availability to minimize oreliminate downtime.6. Ability to inform GHSC-PSM at least ten (10) days ahead prior to any monthly scheduledmaintenance or twenty-four (24) hours ahead prior to unscheduled emergency maintenance.The table below contains the list of services under RFQ. Offerors are requested to providequotations containing the information below on official letterhead or official quotation format.
