DIRECTORATE OF BOILERS User Registration & Application Request Manual
Introduction Purpose of this user registration manual is to explain the user registration steps on the http://db.mp.gov.in/. When user open this web site they have the option to register their profile, once they register their profile successfully they can login with their registered profile and track the status of the application request they have submitted. This document explain the steps for the user registration and managing their profile and requests over the portal. Step 1: User Registration To register the profile with Directorate of Boiler unregistered firms need to select the New User Registration option from the home page and provide following details. 1. Firm Details: Under this section firm details need to be provided by the unregistered firms, following fields are mandatory for them to provide. a. Name of Firm b. Firm Type c. PAN d. Telephone Rest of the fields on the form are optional. Page 3 of 27 Directorate of Boilers Confidential
2. 3. Address Details: Unregistered firm need to provider their address details for the registration over the portal, following fields are mandatory for them to provide. a. Block No/House No/Plot No b. Road/Street/Lane c. Area Locality d. Pin Code e. District Board Member Details: Under this section board member details needs to be provided by the firm, following fields are mandatory for them to provide. a. Members Name b. Gender c. Category d. Knowledge Level e. Equity Participation ( in ) f. Equity Participation (in %) They can add multiple Board member details under this section. 4. Contact Person Details : Under this section contact person details needs to be provided by the firm, following fields are mandatory for them to provide a. Representative Name b. Mobile Number c. Email Id 5. Login Details: Under this section firm can select their preferred login name to access their profile, this user name should be unique and system will show them a notification message for the case if entered user name is already registered in the application database. 6. Once after these details are submitted over the portal a system generated password will be sent on the provided email id (under contact person detail section). 7. Existing registered user of the portal can login with their registered credentials from the “Login to Existing User” option from the home page. Page 4 of 27 Directorate of Boilers Confidential
8. Once they click on the login option they redirected to the login screen 9. For the case if registered user forget their password they can recover their password by clicking on the recover password option from the login screen 10. To recover the password they need to provider their registered user name and new system generated password will be sent on the registered email id of the user. 11. After successful login registered user can also change their password from the option available under the user profile from the top right corner Once they click on this option they will be redirected on the below screen and they can change their existing password. Step 2: Dashboard 1. Once after successful registration of the profile user can access their profile, after login they will be redirected to the dashboard of the portal where they can view the status of the application request they have submitted. 2. They can manage their password and their password from the change password option under their profile from the top right corner. 3. From the left menu panel they can navigate on the applications forms as listed below. a. Boiler Registration b. Transfer of Boiler c. Annual/Renewal Inspection d. Repair/Modification Inspection e. Step 3: Boiler Registration 1. To register a new boiler logged in user need to select the Boiler Registration option from the left menu. 2. They need to select the type of the boiler user want to register a. Boiler Page 8 of 27 Directorate of Boilers Confidential
b. Small Industrial Boiler c. Economizer Based on the selected type of the boiler user need to upload the document under Undertaking and Enclosures section. 3. Once after the selection of the boiler type user need to provide following details a. Boiler Details : User need to provide following details of the boiler i. Types of Boiler/ Small Industrial Boiler/ Economizer ii. Total Heating Surface Area (Sq. mt) iii. Name of Manufacturer iv. Boiler/ Small Industrial Boiler/ Economizer Works Number v. Manufacturing Place vi. Manufacturing Year vii. Maximum Evaporation (Tonne/Hr) viii. Maximum Working Pressure (Kg/cm2) ix. Power Generation in MW x. Maximum Working Temperature in C xi. Is Piping Involved (Yes/No) b. Registration Fee Details : User need to provide fee details for the registration of the boiler i. Fee Amount ii. Bank Name iii. Challan Number iv. Date Of Payment c. Scrutiny & Inspection Fee Details for Pipe Line : User need to provide the fee details for the scrutiny & inspection i. Fee Amount ii. Bank Name iii. CIN/CRN/Challan Number iv. Date of Payment d. Undertaking and Enclosures : Based on the selected type of the boiler registration user need to upload the following document under this section i. For the case when user select boiler only they have to upload following documents, all of these documents are mandatory to upload. 1. Form 2D 2. Registration Fee Challan 3. ii. For the case when user select Small Industrial Boiler they have to upload following documents, all of these documents are mandatory to upload. 1. Form 2D 2. Registration Fee Challan 3. IBR Certificate form : II (1) or II (2) 4. Form XVII Page 10 of 27 Directorate of Boilers Confidential

e. Once they provide all of these details user need to click on the check box of undertaking that all of these details provided by them are true and correct. 4. Once they submit these form application request will be shared with the Directorate of Boiler for their review and approval. 5. User can track the status of the application from the dashboard. Step 4: Transfer of Boiler 1. For the case when any registered boiler move from other State to Madhya Pradesh then user need to initiate request for the Transfer of Boiler. 2. To initiate the transfer request logged in user need to select the type of the registration of the boiler and they have to enter the registration number of the boiler along with the following details a. Boiler Details : User need to provide following details of the boiler i. Types of Boiler/ Economizer ii. Total Heating Surface Area (Sq. mt) iii. Name of Manufacturer iv. For the case when user select registration type as boiler it’s mandatory to upload following documents. 1. Form 2D 2. Inspection Fee Challan 3. Certificate of Welder 4. Details of Supervisor Rest of the documents are optional to upload 1. Steam pipe line drawing duly signed by owner 2. Scrutiny & HT fee by Challan 3. ii. For the case when user select registration type as economizer its mandatory to upload the following document under this section 1. Form 2D 2. Inspection Fee Challan 3. Certificate of Welder 4. Details of Supervisor Rest of the documents are optional to upload 1. Steam pipe line drawing duly signed by owner 2. Scrutiny & HT fee by Challan 3. Registration certificate of contractor e. Once they provide all of these details user need to click on the check box of undertaking that all of these details provided by them are true and correct. 3. Once they submit these form application request will be shared with the Directorate of Boiler for their review and approval. 4. User can track the status of the application from the dashboard. Step 5: Annual/Renewal Inspection 1. This application request allow the user to initiate the request for the annual /renewal inspection of the registered boiler 2. To initiate the annual /renewal inspection request logged in user need to select the type of the registration of the boiler and they have to enter the registration number of the boiler along with the following details a. Boiler Details : User need to provide following details of the boiler i. Types of Boiler/ Economizer ii. Name of Manufacturer iii. Boiler/ Economizer Works Number iv. Manufacturing Place v. Manufacturing Year vi. Maximum Evaporation (Tonne/Hr) vii. Maximum Working Pressure (Kg/cm2) viii. Power Generation in MW ix. Total Heating Surface Area (Sq. mt) x. RLA Status xi. b. Inspection Fee Details: User need to provide the fee details for the inspection i. Fee Amount ii. Bank Name iii. CIN/CRN/Challan Number iv. Date of Payment c. Undertaking and Enclosures : Based on the selected type of the boiler registration user need to upload the following document under this section i. For both of the cases when user either select registration type as boiler or economizer it’s mandatory to upload following documents. 1. Form 2D 2. Inspection Fee Challan d. Once they provide all of these details user need to click on the check box of undertaking that all of these details provided by them are true and correct. 3. Once they submit these form application request will be shared with the Directorate of Boiler for their review and approval. 4. User can track the status of the application from the dashboard. Step 6: Repair/Modification Inspection 1. This application request allow the user to initiate the request for the Repair & Modification Inspection of the registered boiler 2. To initiate the Repair & Modification inspection of the registered boiler logged in user need to select the type of the registration of the boiler and they have to enter the registration number of the boiler along with the following details a. Maximum Working Pressure (Kg/cm2) ix. Tentative Inspection Date x. Total Heating Surface Area (Sq. mt) xi. Maximum Temperature in C xii. Power Generation in MW xiii. Is Piping Involved ( Yes/No) b. ii. For the case when user select registration type as economizer it’s mandatory to upload following documents. 1. Form 2D 2. Inspection Fee Challan 3. Certificate of Welder 4. Details of Supervisor Rest of the documents are option to upload 5. Steam pipe line Drawing Duly Signed by Owner 6. Scrutiny & HT Fee by Challan 7. Registration Certificate of Contractor 8. e. Once they provide all of these details user need to click on the check box of undertaking that all of these details provided by them are true and correct. 3. Once they submit these form application request will be shared with the Directorate of Boiler for their review and approval. 4. User can track the status of the application from the dashboard. Step 7: Third Party Inspection 1. This application request allow the user to initiate the request for the Third Party Inspection of the registered boiler 2. To initiate the Third Party inspection of the registered boiler logged in user need to select the type of the registration of the boiler and they have to enter the registration number of the boiler along with the following details a. Boiler Details : User need to provide following details of the boiler i. Types of Boiler/ Economizer ii. Expiry date of P.O./Certificate iii. iv. Name of Manufacturer v. Manufacturing Place vi. Manufacturing Year vii. Maximum Evaporation (Tonne/Hr) viii. Maximum Working Pressure (Kg/cm2) ix. Date of Inspection x. Power Generation in MW xi. Name of the Competent person b. Inspection Fee Details: User need to provide the fee details for the inspection i. Fee Amount ii. Bank Name iii. CIN/CRN/Challan Number iv. Date of Payment c. Undertaking and Enclosures : Based on the selected type of the boiler registration user need to upload the following document under this section i. Below document is mandatory to upload irrespective of boiler registration type selected by the user 1. Inspection Fee Challan Rest of the documents are option to upload 2. Certificate Issued by Competent Person d. Once they provide all of these details user need to click on the check box of undertaking that all of these details provided by them are true and correct. 3. Step 8: Registration/Renewal for Manufacturer/Repairer/Erector 1. This application request allow the user to initiate the request for the Registration/Renewal of Manufacturer /Repairer/Erector. 2. To initiate the application request logged in user need to select the type of the application i.e. they want place the request for Manufacturer or Repairer or Erector, they can select multiple option from the multi select drop down. They also need to provide following details a. Firm Details : User need to provide following firm details i. Firm Name ii. Firm Address iii. Owner name iv. Year Of Establishment v. Class b. Deposited Fee Details: User need to provide the fee details for the inspection i. Fee Amount ii. Bank Name iii. CIN/CRN/Challan Number iv. c. Undertaking and Enclosures :User also need to upload the following document under this section , all documents are mandatory to upload i. Fee Challan ii. Form XVIII iii. Details of Machinery iv. To initiate the Boiler /Economizer Steam Test of the registered boiler logged in user need to select the type of the registration of the boiler and they have to enter the registration number of the boiler along with the following details a. Boiler Details : User need to provide following details of the boiler i. Types of Boiler/ Economizer ii. RLA Status iii. Boiler/ Economizer Works Number iv. Name of Manufacturer v.
