Purpose of the Request for ProposalThe City of Chicago ("City"), acting through its Police Department ("CPD"), invites thesubmission of proposals from firms with expertise and experience providing PsychologicalPre-Employment Assessment Services Testing, Psychological Fitness for Duty AssessmentServices, and/or Psychological Re-Evaluation Testing (the “Services”).The Selected Respondent (“Contractor”) awarded a contract pursuant to this RFP shallperform all applicable duties as outlined in Exhibit 1, the Scope of Services.Respondents may submit a proposal for a single Group, as well as for multiple Groups, andmay select all three Groups. The submitted proposal should contain a complete responseand pricing addressing each particular Group for which Respondent is applying. The City willselect up to two and as many as three Respondents for the three Groups in order to avoidany potential conflicts of interest during the contract period.The City recognizes that certain conflicts of interest may occur if, for example, the Contractorthat performed the services for Group A or Group B also performed the Group C(Psychological Re-Evaluation) services. The Contractor must not be in a position to reevaluate its own work. As a result, these services will be performed by separate Contractors.The work contemplated is professional in nature. It is understood that the Contractor actingas an individual, partnership, corporation or other legal entity, is of professional status,licensed to perform in the State of Illinois and licensed for all applicable professionaldiscipline(s) requiring licensing and will be governed by professional ethics in its relationshipto the City. It is also understood that all reports, information, or data prepared or assembledby the Contractor under a contract awarded pursuant to this RFP are confidential in natureand will not be made available to any individual or organization, except the City, without theprior written approval from the City.The Contractor shall be financially solvent and each of its members, if a joint venture, itsemployees, agents or subcontractors of any tier shall be competent to perform the servicesrequired under this RFP document.B. LaSalle Street in Chicago, IL 60602.A Respondent who chooses to download an RFP solicitation instead of picking it up in personwill be responsible for checking the aforementioned web site for clarifications and/oraddenda, if any. Failure to obtain clarifications and/or addenda from the web site shall notrelieve Respondent from being bound by any additional terms and conditions in theclarifications and/or addenda, or from considering additional information contained therein inpreparing your Proposal. Note, there may be multiple clarifications and/or addenda. If Respondentchooses to download and print an RFP document, the Respondent must contact theCity of Chicago, Department of Procurement Services, Bid & Bond Room by emailingBidandBond@cityofchicago.org to register Respondent’s company as an RFPdocument holder, which will enable the Respondent to receive any future clarificationsand/or addendum related to this RFP.The City accepts no responsibility for the timely delivery of materials or for alertingRespondents on posting to the DPS website information related to this RFP.II.DEFINITIONSThe following defined terms have the meanings specified below.NOTE: If a defined term is used only once, then it may be defined within the context in whichit appears in that paragraph or section.“Addendum” means a revision of the RFP Documents issued by the Chief ProcurementOfficer prior to the due date for submitting proposals.“Agreement” means the City of Chicago’s Professional Services Agreement, including allexhibits attached to it and incorporated in it by reference, and all amendments, modifications,or revisions made in accordance with its terms, as attached in this RFP in Exhibit 9.“Chief Procurement Officer” or “CPO" means the Chief Procurement Officer of the City ofChicago and any representative duly authorized in writing to act on her behalf.“Clarification” means the City’s response to questions submitted by Respondents which donot revise the requirements of the RFP.“Commissioner” or “Superintendent” means the Superintendent of the City of ChicagoPolice Department, and any representative authorized in writing to act on the CitySuperintendent’s behalf.“Contractor” or “Consultant” means the Respondent awarded a contract pursuant to thisRFP process.2

RFP for Psychological Testing for Pre-Employment, Fitness for Duty and Re-EvaluationSpecification No. 886127“Department” or “CPD” means the City of Chicago Police Department.“EC” means the City Evaluation Committee appointed to review and assess all Proposals,and make its recommendations to the Commissioner concerning its evaluations.“Proposal” is the written content and documents submitted by a Respondent in response tothis RFP.“Respondent” means the primary entity which submits a Proposal in response to this RFPand may include subcontractors and other affiliates.“Selected Respondent” means the awardee of the contract from this RFP.“Services” means performance of all tasks, activities and deliverables as described in detailin Exhibit 1, Scope of Services and performed by qualified and licensed personnel of theselected Contractor from this RFP.III.SCOPE OF SERVICESA.Description of ServicesThe services that the City seeks to acquire are described in detail in the Scope of Services,Exhibit 1.B.Contract TermAny contract awarded pursuant to this RFP solicitation shall be for a base contract periodof five (5) years plus three (3) additional (1) year extension options.IV.GENERAL INFORMATION AND GUIDELINESA.
