Topic No. 500-000-015-iLoss Prevention ManualEffective: November 18, 2010INTRODUCTIONLOSS PREVENTION MANUALPURPOSE:To establish a Department uniform loss prevention program designed to reduce and/oreliminate the number of work-related injuries to employees and damage to materials orproperty due to injuries, illness, vehicle crashes, and incidents. Also, to establishuniform operational plans and delineate responsibilities for maintaining a safe workingenvironment for all employees of the Department.AUTHORITY:Sections 20.23(3)(a), Section 284.50, and 334.048(3), Florida Statutes (F.S.)SCOPE:This Loss Prevention Manual (Manual) applies to all employees and operations of theDepartment.REFERENCES:The Governor’s Executive Order, No. 2000-292, directed all State agencies in Floridato comply with Subparts C through T and Z of the Occupational Safety and Health(OSHA) Subparts, 29 CFR Part 1910, as revised July 1, 1993. The Department alsorequires employees to comply with the most current OSHA standards found in 29 CFRParts 1926, and 1928.51.GENERAL:(1)The Department’s loss prevention program will be implemented through safetymanuals and guidelines promulgated by the State Safety Office based upon therequirements set forth by the state and federal agencies having such authorityand jurisdiction.(2)The Department’s facilities will be planned with the intent of providing eachemployee a place of employment free from recognized hazards. Thiscommitment requires that safety be considered in all phases of operations.(3)The State Safety Office shall provide the leadership and technical assistance todevelop implementation strategies and policies to promote safety awareness,hazard recognition and correction, and reporting within the Department.Loss Prevention ManualIntro-1

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010Chapter 1RESPONSIBILITIES1.1DEPARTMENTThe Department shall furnish a work environment that is safe for itsemployees and shall adopt measures reasonable and necessary toprotect the life, health, and safety of its employees.1.2DISTRICT SECRETARIES AND THE CENTRAL OFFICEEach District Secretary, or designee, and a representative from theCentral Office shall establish a loss prevention program that includes, at aminimum, the program elements specified in this chapter. Theseindividuals shall ensure that program and processes identified in thismanual are implemented and a process is in place to assure eachemployee is in compliance.1.3UNIT MANAGERS/OFFICE HEADS1.3.1A Unit Manager/Office Head is an individual who exercises direction andcontrol over one or more cost centers.1.3.2Each Unit Manager/Office Head is responsible for enforcing all safetyrules and regulations and for implementing loss prevention programs.They are also responsible for ensuring their employees are provided withrequired safety training in accordance with Chapter 12 of this Manual.1.3.3Unit Managers/Office Heads responsible for promulgating proceduresshall ensure that such procedures address safety-related issues andresponsibilities in compliance with applicable federal, state, and otherregulations and industry standards.1.3.4The Unit Manager/Office Head may designate and assign specific safetyresponsibilities to an employee in implementing the requirements of thismanual.1.4IMMEDIATE SUPERVISORS1.4.1The immediate supervisor is an individual who exercises direction andcontrol over the work activity of one or more employees.Loss Prevention Manual1-1

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 20101.4.2The immediate supervisor in charge of any work activity is responsible forensuring that a safe work environment is maintained, safe work practices are followed,and employees are trained. No employee will be required to do a job using unsafeequipment or that violates safe work practices.1.4.3The immediate supervisor is responsible for notifying employees to beresponsible for their own safety and the safety of their peers. They arealso responsible for assessing and reducing work related risks.1.5SAFETY AND HEALTH MANAGERS AND SPECIALISTS, ORDESIGNATED SAFETY REPRESENTATIVE1.5.1Safety and Health Managers or Other Designated RepresentativeThe Safety and Health Managers or designees are the focal point forcoordination of his/her District safety and health programs. Theirresponsibilities include: implementation of loss prevention programs,identifying appropriate training programs for employees, and providingtechnical assistance and guidelines for District operations of Safe WorkPractices.1.5.2Safety and Health Specialists or Designated Safety RepresentativeSafety and Health Specialists or designated safety representativesfunction as technical support staff for District Safety and Health Managersor designee and Unit Managers in implementing and monitoringcompliance with safety and health programs in their assigned units orareas of responsibility. They may be assigned to field offices or forindividual District departments or divisions. Their main functions are tocoordinate safety meetings and conduct self inspections of their areas ofresponsibility (this may not apply to each District.)1.5.3Central OfficeCentral Office does not have a Safety and Health Manager or Specialistlike the District Offices. Unit Managers/Office Heads or their designatedSafety Representatives are responsible for safety and health matters intheir work units, including reporting of injuries/illnesses orcrashes/incidents. Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010procedural assistance, training guidelines, statistical data, awarenessmaterials, and other resources as identified in Section 1.7 of this Manual.1.6EMPLOYEES1.6.1It is the responsibility of each employee to comply with established policies,procedures, and safe work practices.1.6.2Employees shall immediately report to their immediate supervisors anyunsafe work practices or unsafe conditions, either verbal or in writing,such as:(A)Unsafe condition(s) of motor vehicles, equipment, facilities, shops, orproperty owned, leased, or operated by the Department where conditionsmay jeopardize the safety of the employee, other employees, or thepublic;(B)Any practice or operation being carried on by Department employees thatmay jeopardize the safety of the employee, other employees, or thepublic;(C)Any practice or operation being carried on by non-Department employeesthat may jeopardize the safety of Department employees while performingtheir assigned work.1.6.3Employees shall report any work-related incident resulting in personalinjury or illness, including any crash or incident involving a Departmentvehicle, to their immediate supervisor or other employees designated bythe Unit Manager/Office Head before the end of the employee’s shift.1.6.4Employees who report unsafe acts or conditions to their immediatesupervisor shall not be harassed for fulfilling their reportingresponsibilities.1.7STATE SAFETY OFFICE1.7.1The State Safety Office shall establish procedures, standards, and trainingrequirements concerning the loss prevention program, in accordance withSection 284.50, F.S.1.7.2The State Safety Office shall review procedures promulgated by otherDepartment offices to ensure that safety-related issues andLoss Prevention Manual1-3

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010responsibilities have been addressed in a manner that is in compliancewith applicable regulations and industry standards, and to ensurecompatibility with the provisions of this manual.1.7.3The State Safety Office shall be responsible for the development of safework practices that must be observed by employees in performing theirduties. The State Safety Office will update the manual as needed. TheState Safety Office will provide guidelines for training on safe workpractices when appropriate.1.7.4The State Safety Office may conduct periodic quality assurance reviews toassess the progress of loss prevention programs and to monitor theachievements to reach desired goals and/or objectives. Results of thereview shall be furnished to the respective Unit Manager/Office Head.1.7.5The State Safety Office shall provide technical assistance and guidelinesfor training to the Districts and Central Office in developing andimplementing their programs.1.8HAZARD REPORTING SYSTEM “HOTLINE”1.8.1Hazard recognition and reporting are integral parts of an effective lossprevention program and are the responsibility of all Departmentemployees.1.8.2All Department employees are encouraged and expected to report anysuspected hazard to supervisors or appropriate unit personnel, or byutilizing the Hazard Reporting System Hotline. The “Hotline” is availableto all Department employees who believe that an immediate hazardexists, and/or who may wish to report such hazards while remaininganonymous.1.8.3Hazards can be reported by calling the “Hotline” at (850) 414-5255. Anyhazard reported utilizing the “Hotline” shall receive immediate action, andwill remain confidential.Loss Prevention Manual1-4

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010Chapter 2TRAINING AND EDUCATION2.1TRAINING AND EDUCATION GOALSSafety education and training programs have been established to motivateand train employees in recognition, avoidance, and prevention of unsafeacts and unsafe conditions while performing assigned tasks.2.2SAFETY ORIENTATIONSafety orientation includes general information about safety. All newlyhired employees shall be given an orientation to make them aware of theimportance of safety and their responsibility for maintaining a safe workenvironment. Appropriate safety personnel or designated person shallconduct safety orientations within thirty (30) working days of employment.Safety orientation presents general information about safety policies,safety procedures, rules, and regulations and safety awareness,document Safety Orientation in TRESS using Course Number ST-090197.2.3SAFETY INDOCTRINATION2.3.1Safety indoctrination is conducted for job specific safety training. All newlyhired, reassigned, or promoted employees shall be given a safetyindoctrination to ensure complete understanding of their job functions andthe Department's safety policies and procedures including job-specificsafety instructions. The employee’s immediate supervisor shall conductindoctrination within five (5) working days of employment, reassignment,or promotion (TRESS Course Roster (without PDH), Form No. 260-02003).2.3.2Indoctrination will not be required when an employee is reassigned and/orpromoted to a position where he or she has undergone indoctrinationwhile in the previous position.2.3.3.1Upon completion of the Safety Indoctrination, Form No. 500-000-16,must be completed and signed by the employee, with a copy placed in theunit file.Loss Prevention Manual2-1

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010Chapter 3INCIDENT REPORTING AND INVESTIGATION PROCESS3.1REPORTING OF INCIDENTS3.1.1All accidents that result in personal injury, illness, or property damageshall be immediately reported and investigated, regardless of the extent ofinjury, illness, or property damage. Employees must report incidentsbefore the end of the employee’s shift to their immediate supervisor.Non-fatal incidents are reported by using the appropriate forms (Form No.500-000-18, Injury/Illness Report, or 500-000-15, VehicleCrash/Incident Report). See Chapter 4, Personal Injury/IllnessReporting; and Chapter 5, Vehicle Crash/Incident Reporting for furtherdetails.3.2REPORTING OF FATALITIES3.2.1Any incident that results in the fatality of any Department employee shallbe reported to the Unit Manager or his/her designee who will contact theappropriate Personnel Officer, District Industrial Safety Office, District PIO,and the State Safety Office, Industrial Safety.3.2.2Any incident which results in an employee related work activity, fatality onfacilities or properties owned, leased, or regulated by the Department mustbe reported immediately to the respective District Secretary or AssistantSecretary.3.3INVESTIGATION OF ACCIDENTS3.3.1The purpose of an incident investigation is to gather information andrecord facts about the incident that caused the injury, illness, or propertydamage and educate and train those involved and to prevent futurerecurrences.3.3.2The investigation should:(A)Identify the primary cause(s) and/or contributing factors leading to theaccident;(B)Determine what, if any, work practices or procedures are involved in theaccident;Loss Prevention Manual3-1

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010(C)Determine what corrective actions can be taken to prevent similaroccurrences;the(D)Gather all related information needed to answer the question of whathappened, to whom, when, and where;(E)Identify person(s) involved in the incident and person(s) who witnessedoccurrence;(F)Include an interview of each witness, if possible, which might provideinformation on the underlying cause(s) of the incident.3.4INFORMATION GATHERING3.4.1The primary responsibility for conducting the investigation and gatheringneeded information about the incident rests with the immediate supervisorof the employee involved in the accident or with any other employeedesignated by the Unit Manager/Office Head for this purpose.3.4.2The investigation of the accident should be initiated immediately. Therequired reports, Form No. 500-000-18, Injury/Illness Report and FormNo. 500-000-15, Vehicle Crash/Incident Report should be completedwithin seven (7) working days of the occurrence.3.4.3All information gathered from the investigation shall be recorded on therequired report form. The report will provide findings regarding theaccident to the Unit Manager/Office Head and provide a written record ofthe interim action(s) or corrective action(s) being taken to prevent similaroccurrence(s).3.5REVIEW OF INJURY/ILLNESS AND VEHICLECRASH/INCIDENT OCCURRENCES3.5.1The Unit Manager/Office Head may designate a group of employees toreview injury/illness and crash/incident occurrences for the purpose of:(A)Evaluating the interim action(s) or corrective action(s) taken to preventsimilar occurrences;(B)Recommending permanent action(s) to eliminate or reduce the risk ofrecurrence.Loss Prevention Manual3-2

Topic No. 500-000-015Loss Prevention Manual3.6Effective: November 18, 2010DRIVER RECORD CHECKSAll Department employees will have their driver records checked monthlyby the State Safety Office, and any unacceptable driving records will besent to the Districts and to appropriate cost center managers in theCentral Office. It will then become the managers’ responsibility to take theappropriate action based on the employee’s need to drive, either toperform the job duties of the position or occasionally to performDepartment’s business, in accordance with the requirements of Driver’sRecord Requirements, Procedure No. 250-000-010.Loss Prevention Manual3-3

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010Chapter 4PERSONAL INJURY/ILLNESS REPORTING4.1RESPONSIBILITIES4.1.1The Employee shall:The employee who is injured or becomes ill as a result of a work-relatedaccident shall immediately report the accident to his/her immediatesupervisor or designee before the end of the employee’s shift. Theemployee must complete Section A and B of the Injury/Illness Report,Form No. 500-000-18. If available the employee must sign Section B ofthe form.4.1.2The Immediate Supervisor shall:(A)Ensure that the employee is provided appropriate first aid or medicaltreatment, as needed, in accordance with Procedure No. 250-032-001,Workers’ Compensation. Universal precautions must be observed asdescribed in the Bloodborne Pathogens Exposure Control Plan,Chapter 13, and Section 13.7 of this manual.(B)Call the Managed Care Provider (contact your Workers’ CompensationCoordinator for the current phone number) who will provide instructionswhere the injured employee should go for medical treatment.(C)As soon as the needs of the injured or ill employee are taken care of,conduct an investigation of the incident, and fill out Sections C and D (1)of the Injury/Illness Report, No. Form 500-000-18(D)Forward the report to the Unit's Safety and Health Specialist or designee.4.1.3The Unit's Safety and Health Specialist or designee shall:(A)Provide assistance, if needed, in investigating the incident.(B)Fill out Section D (2) of the Injury/Illness Report (Form No. 500-00018), indicating recommendations for preventive action(s).(C)Forward the report to the Unit Manager/Office Head.Loss Prevention Manual4-1

Topic No. 500-000-015Loss Prevention Manual4.1.4The Unit Manager/Office Head shall:(A)Review the report and complete Section D (3) of Form No. 500-000-18indicating the corrective action(s) to be taken.(B)Forward a copy of the completed report to the District Safety and HealthManager or designated person. For Central Office, the completed reportshould be forwarded to the State Safety Office, Industrial Safety, MS 53.(C)The information shall be input into Industrial Safety InformationManagement System within 30 calendar days by the authorizeddepartment.4.1.54.2Effective: November 18, 2010The District Safety and Health Manager or designee shall:(A)Provide assistance, if needed, in implementing the preventive/correctivemeasures or corrective action(s).(B)Review all reports and provide, when appropriate, recommendations tothe Unit Manager/Office Head on how to prevent similar occurrences.FORMForm No. 500-000-18, Injury/Illness Report, is available from theDepartment’s Forms Library.Loss Prevention Manual4-2

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 2010Chapter 5VEHICLE CRASH/INCIDENT REPORTINGPROCESS5.1PURPOSEAll crashes and incidents involving Department motor vehicles, heavyequipment, motorized off-road equipment, watercraft, and aircraft, andleased, rented, and privately owned vehicles used for official businessshall be reported on Form No. 500-000-15, Vehicle Crash/IncidentReport.5.2DEFINITIONS5.2.1Vehicle Crashes – A crash involving at least one motor vehicle whichresults in a fatality, injury, or property damage when such vehicle is inoperation on a traffic lane. Any vehicular thoroughfare open to the public,is considered a traffic lane. Vehicle crashes also include:(A)Crashes involving off-road motorized equipment being operated on atraffic lane.(B)Crashes involving a privately owned vehicle, or a leased or rented vehicleused by a Department employee on official business.(C)All backing incidents.Vehicle Incidents – Vehicle incidents involving:5.2.2(A)Damage to off-road motorized equipment not being operated on oradjacent to the traffic lane.(B)Damage to windshields, windows, signal lights, headlights, or tail-lightscaused by tools, branches, debris, or similar objects.(C)Any damage caused to other property by unsecured items falling out ofvehicles.(D)Any damage caused to other property by objects thrown during mowingoperations.(E)Any damage to vehicles being operated off-road or in a designated Workzone.Loss Prevention Manual5-1

Topic No. 500-000-015Loss Prevention ManualEffective: November 18, 20105.2.3Chargeable Crash/Incident – Any crash or incident caused bynegligence, failure to abide by laws, rules, procedures, or generallyaccepted by safety practices. The determination of “Chargeable” isexclusive of any police agency reports or citations. This determination isbased on a causal analysis.5.3VEHICLE CRASH5.3.1In the event of a vehicle crash, the driver shall:(A)Make every effort to have the vehicle moved out of the normal flow oftraffic unless the crash results in death, personal injury, or extensivedamage to the vehicle so that it cannot be moved. Under theseconditions, the vehicle shall not be moved unless directed by lawenforcement or other authority.(B)Notify his/her immediate supervisor or other employees designated by theUnit Manager/Office Head before the end of the employee’s shift. Followthe instructions found on Page 5 of the Vehicle Crash/Incident Report,Form No. 500-000-15. Also, follow the “Driver Instructions in the Case ofVehicle Crash” located in the glove compartment of the vehicle.(C)Call law enforcement (FHP first) to report the accident. If law enforcementwill not respond because there is minimal or no damage and there wereno injuries, then request an accident case number and include thisnumber on the crash report.(D)Fill out Sections A, B, C, D, and E of the Vehicle Crash/Incident Report,Form No. 500-000-15 and submit the report to his/her immediatesupervisor before the end of the employee’s shift. If the driver is on travelstatus, then report to the immediate supervisor upon arrival at his/herofficial work location or destination.The immediate supervisor shall:5.3.2(A)Review the report and ensure that the driver's account and details of thecrash are recorded. Conduct a fact finding investigation of the crash todetermine causes and fill out Sections F and G (1) of the report. (See the contact list from thevehicle safety package.)5.3.3The Unit Safety and Health Specialist or designee shall:(A)Assist the immediate supervisor, if needed, in conducting fact findinginvestigations and completing reports.(B)Ensure that the driver and the immediate supervisor have filled out theappropriate sections of the Vehicle Crash/Incident Report Form No.500-000-15, and that copies of all relevant documents are attached.(C)Fill out Section G (2) of the report, including comments andrecommendations.(D)Submit the report to Unit Manager/Office Head.5.3.4The Unit Manager/Office Head shall:(A)Review the report and fill out Section G (3) with comments onpreventative and corrective action(s) to be taken. Forward the report tothe District Secretary/Assistant Secretary or designee. The designee mayalso be a District Safety Management Review Committee or DepartmentHead as designated by the District Secretary.(B)After review by the District Secretary, or designee or Assistant Secretaryand/or completion of the report, the Unit Manager will identify, to theimmediate supervisor, the corrective actions to be taken.
