Activation of CoverIn order for You to receive cover under this Policy forYour Journey, You must: Continue to be a current holder of a Platinum cardaccount, and On or after 23 October 2010 and prior to thecommencement of Your Journey, purchased withYour Platinum account a minimum of 250 forYour pre-booked transport costs (airfares and/orcruise) and/or land content (tours, hire cars orother hired transport and accommodation), and This payment must be debited to Your accountprior to Your departure from Australia. We will notrespond to any claim until We have receivedproof of this payment by You, and You hold a ticket to return You to Australia, andthat return must be within six months of thecommencement of Your Journey. Should You hold a date changeable ticket and Yousubsequently decide to amend Your itinerary thatchanges Your travel duration to beyond six months,all cover under this policy will cease immediately fromthe time You make the change to Your itinerary.2. The Policy is a contract of insurance between theANZ and Us. It is not a contract between Chartisand You.3. Age Limit Your cover under this Policy is conditional uponYou being 80 years of age or under at the time Youactivated Your cover.4. Your cover under this Policy is conditional uponYou being a permanent resident of Australiaintending to conclude Your Journey in Australia.5. Pre-existing Medical Condition(s) This policy does not provide any benefits for anyPre-existing Medical Condition(s) You have or anyPre-existing Medical Condition(s) of a Relative,Travelling Companion or any other person thatmay give rise for You to claim. Please refer to thedefinition section and General Policy Exclusionssection for further information. If You wish to apply for cover for Your Pre-existingMedical Condition(s) please contact Travel Guardbefore You leave Australia, on 1800 009 391. If Weagree to cover Your Pre-existing MedicalCondition(s), Travel Guard will send You a letterconfirming that Your Pre-existing MedicalCondition is covered under this policy for theJourney in question and whether the cover issubject to any special conditions.If We agree to cover some or all of Your Pre-existingMedical Conditions an administration fee of 75 perperson is payable to cover the cost of the assessment.6. We will not pay claims resulting from Luggage LeftUnsupervised in Public Place.7. If You have a loss, You must tell Us in writing within30 days of returning from Your Journey. If YourJourney is cancelled, You must write to Us within30 days of the cancellation. We will give You a claimform, which You must complete and send back toUs with information which supports Your loss. Please refer to the “How to make a Claims” sectionfor full details.8. You should consider whether the benefits offeredunder the Platinum Overseas Travel and MedicalInsurance Policy are appropriate for Yourcircumstances. The terms and conditions under thisPolicy cannot be amended or negotiated by You.Privacy consent and disclosureAmerican Home Assurance Company (“AHAC”)trading in Australia as Chartis (“Chartis”) is bound bythe National Privacy Principles that apply to anypersonal information collected by Chartis.Purpose of CollectionChartis collects information necessary to underwrite andadminister Your insurance cover, to maintain and toimprove customer service. You have a duty under theInsurance Contracts Act to disclose certain information.Failure to comply with Your duty of disclosure or toprovide certain information may result in Chartis eitherdeclining cover, cancelling Your insurance cover orreducing the level of cover.In the course of administering this policy We maydisclose Your information to:(i) any entity to which AHAC is related (whether inAustralia or overseas), contractors or third party3402 CF PLATINUM INSURANCE POLICY T'S & C'S BROCHURE 55983.indd 1024/09/10 11:06 AM

10–11providers providing services related to theadministration of this policy.(ii) banks and financial institutions for the purpose ofprocessing Your application and obtaining policypayments.(iii) assessors, third party administrators, emergencyproviders, retailers, medical providers and travelcarriers, in the event of a claim.(iv) Our assistance provider who will record all calls to theassistance service provided under this policy forquality assurance training and verification purposes.In some circumstances Chartis is entitled to disclose Yourpersonal information to third parties without Yourauthorisation such as law enforcement agencies orgovernment authorities.Access to Your informationYou may gain access to Your personal information bysubmitting a written request to Chartis.In some circumstances, Chartis may not permit access toYour personal information. Circumstances where accessmay be denied include where it would compromise theprivacy of other individuals, or where it would beunlawful.ComplaintsChartis has also established an internal disputeresolution process for handling customer complaints.If You feel You have a complaint about Chartis’compliance with the National Privacy Principles,require assistance in lodging a privacy complaint orYou wish to gain access to the information, You maywrite to The Privacy Manager, Chartis, Level 19, 2 ParkStreet, Sydney, or e-mail r complaint will be reviewed and You will beprovided with a written response. If it cannot beresolved, Your complaint will be referred to Chartis’Internal Disputes Resolution Committee who willrespond within 15 working days. General Policy Conditions1. General – At the commencement of the Period OfInsurance, You must be medically fit to travel anddo not and could not reasonably know of anyreason why Your Journey may need to becancelled or disrupted.2. Australian Law – This Policy shall be interpreted inaccordance with the law of the State or Territory inAustralia in which You reside and be subject to thejurisdiction of the courts of that State or Territoryin Australia.3. Currency – All amounts shown are in AustralianCurrency. If expenses are incurred in a foreigncurrency, then the rate of currency exchange usedto calculate the amount payable will be the rate atthe time of incurring the expense or suffering a loss.4. Cancellation or amendment of Cover – Chartis andANZ may agree to amend or terminate thisPlatinum Overseas Travel and Medical Insurance atany time. Chartis may also refuse to cover anyindividual Platinum Cardholder. Any amendment, termination or individualtermination must be by written notification fromUs and/or ANZ by ordinary mail to Your last knownplace of residence and will be deemed to takeeffect either:a. on the third day after the date of the notice; orb. in the event You are on a Journey and cannot becontacted, immediately upon Your return toYour place of residence following Your Journey.5. Cover will terminate immediately upon cancellationof Your Platinum account for the following reasons:a. cancellation by You.b. cancellation by ANZ in accordance with its rightsunder the terms of its contract with You for theissue of Your Platinum.6. If You have a loss involving other persons or theirproperty, You must not tell them it was Your faultor that You will pay them for the loss.7. 12–13 You must assist Us even after We have paid Yourclaim if We want to defend You against an allegation.This could include attending court to give evidence.8. Subrogation If You can claim from anyone else and We alsopay You, then You must refund Us the amountWe paid if they pay You. You cannot claim fromUs and them unless We are only making up thedifference. You must assist Us even after We have paid Yourclaim if We want to recover the amount of anypayment from anyone who caused You to sufferloss or damage. This could include attendingcourt to give evidence. You must not start any legal action beforetelling Us.9. Fraudulent Claims – if any claim is in any respectfraudulent or if any fraudulent means or devices areused by You or any one acting on Your behalf toobtain any benefit under this Policy then any amountpayable in respect of such claim shall be forfeited.DefinitionsWords with a special meaning are shown in the policyin bold and in capital letter.ANZ means Australia and New Zealand BankingGroup Limited ABN 11 005 357 522 and its successorsand assigns.Business Partner means a person who is in a legal,commercial partnership with You in Australia. Abusiness partner must be a permanent resident of,and living in, Australia.Cardholder means a primary cardholder of a Platinumcredit card account which has been issued by ANZ.Dental Expenses are costs You incur for emergencyDental Treatment.Dental Treatment – is the emergency treatment ofhealthy natural teeth or gums to stop sudden pain.This does not include normal or ongoing care of teeth.Dentist – is a general practitioner with thequalifications required to practise dentistry.Disablement/Disabled – means, for You or YourTravelling Companion, a serious accident or illness, whichrequires immediate medical treatment or Dental3402 CF PLATINUM INSURANCE POLICY T'S & C'S BROCHURE 55983.indd 1324/09/10 11:06 AM

Treatment by a Doctor or a Dentist. For a Relative orBusiness Partner, it means a life threatening accident orillness.Doctor – is a general practitioner registered to practisemedicine.Financial Default – means insolvency, bankruptcy,provisional liquidation, liquidation, financial collapse,appointment of a receiver, manager or administrator,entry into any official or unofficial scheme ofarrangement, statutory protection, restructuring orcomposition with creditors, or the happening of anythingof a similar nature under the laws of any jurisdiction.Journey – means a continuous and unbroken period of upto six months during which time You are absent fromAustralia or its Territories. The travel arrangements for theJourney must be booked before You leave Australia or itsTerritories and must include a fixed date ticket returningYou to Australia within six months.Luggage – means the personal items You take with Youon Your Journey. This includes items of clothing, personaljewellery, photographic and video equipment, hearingaids, and purchases You make overseas. It does notinclude mechanical or machine parts, items for sale orcargo taken with You or purchased overseas.Luggage Left Unsupervised – means, Your Luggage left:1. with a person other than Your Travelling Companion,or2. in a position where it remains unsupervised forsufficient time for it to be removed without Yourknowledge, or3. at a distance which creates an opportunity for it to betaken without reasonable chance of Youapprehending or identifying the thief.Medical Expenses are costs You incur for MedicalTreatment.Medical Treatment includes, but is not limited to, medicaland surgical care, hospitalisation, medication,physiotherapy, special diet or exercise programs, ongoingassessment or diagnostic investigations.Original Journey – is the Journey You book before Youleave Australia.Period Of Insurance – means, in relation to different typesof cover, the following:3402 CF PLATINUM INSURANCE POLICY T'S & C'S BROCHURE 55983.indd 1424/09/10 11:06 AM

14–151. Cancellation cover under this policy starts from thedate You activated Your cover in accordance with thecriteria in the “Important Matters” section.2. All other covers under this policy start when You leaveYour home in Australia to start Your Journey.3. All cover under this policy stops when the first of thefollowing happens:a. the Journey You booked ends, orb. You return to Your home in Australia, orc. You amend Your itinerary that changes Your travelduration to beyond six months, If something happens for which You can claim underany of the sections “What We will pay for” (other thanLiability), and this causes You to extend Your Journey,cover under this policy will continue until You canreasonably complete Your Journey.Platinum means an original and valid ANZ Frequent Flyer Platinum , or ANZ Rewards Platinum, or ANZ Platinumcredit card account issued to a client of the Australia andNew Zealand Banking Group Limited, declared to becurrent by Australia and New Zealand Banking GroupLimited at the time of any event, injury, loss or damagewhich would allow You to claim under this policy. Youdoes not include an additional or supplementarycardholder unless stated otherwise.American Express is a registered trademark of AmericanExpress. This card is issued by Australia and New ZealandBanking Group Limited pursuant to a license fromAmerican Express.Pre-existing Medical Condition – means, pertaining tothe Platinum Cardholder, any Relative, TravellingCompanion or any other person that may cause You toclaim:a. any physical, medical or dental condition for which,treatment or advice has been received (whether or nota diagnosis has been made), or medication prescribedor taken:i. in the 30 days (or 90 days for persons 75 years ofage or over) before the date on which Your cover isactivated in accordance with the Activation ofCover criteria as stated in the “Important Matters”section.3402 CF PLATINUM INSURANCE POLICY T'S & C'S BROCHURE 55983.indd 1524/09/10 11:06 AM

ii. In the 30 days (or 90 days for person 75 years ofage or over) before Your Journey starts exceptfor Section 2 Cancellation.b. Any chronic or ongoing physical, medical or dentalcondition for which, treatment or advice has beenreceived (whether or not a diagnosis has beenmade), or medication prescribed or taken at anytime before Your cover is activated in accordancewith the Activation of Cover criteria as stated in the“Important Matters” section; orc. Any complication arising from such conditionoutlined above.Only the medical conditions listed below areautomatically covered under this Policy. In any othercircumstance, a Pre-existing Medical Condition(s) coverapplication for additional cover is required. Please referto the “Important Matters” section for further details.ConditionRequirementAsthmaI f no attack requiring treatment bya medical practitioner in the last12 months.CataractsI f You have no ongoingcomplications, are not on a waitinglist for an operation and have notbeen operated on in the last 30days.Diabetes –Non InsulinDependentIf You were diagnosed over 12months ago, and have not had anycomplications in the last 12 months.You must have a blood sugar levelreading between 4 and 10.EarGrommetsWith no current infection.EpilepsyI f there is no underlying medicalconditions and You have notrequired treatment by a medicalpractitioner for a seizure in the last2 years.Gastric Reflux I f the condition does not relate toan underlying diagnosis (ie. herniagastric ulcer).GoutI f the gout has remained stable formore than 6 months.Hiatus Hernia If no surgery is planned.HipReplacementI f performed more than 6 monthsago and less than 10 years ago.3402 CF PLATINUM INSURANCE POLICY T'S & C'S BROCHURE 55983.indd 1624/09/10 11:06 AM

Peptic UlcerI f the condition has remained stablefor more than 6 months.Pregnancyup to andincluding24 weeksIf no complications exist relating tothis pregnancy and the conceptionwas not medically assisted.UnderactiveThyroidIf not as a result of a tumour.16–17Hypertension If You have no heart conditions and(High BloodYour current BP reading is lowerPressure)than 165/95.Public Place –includes but is not limited to shops,airports, train stations, bus stations, streets, hotel foyersand grounds, restaurants, beaches, public toilets, andany places which the public has access. It does notinclude:1. a place where only You, Your TravellingCompanion or Your accommodation providershave access, or2. the storage area of Your accommodation ortransport provider after they have taken YourLuggage from You to place it in safekeeping.Reasonable – means:1. for Medical Expenses and Dental Expenses, thatthe care obtained should be at the standard levelgiven in the country You are in, and must notexceed the level of care You would normallyreceive in Australia.2. for all other expenses, such as transport, meals andaccommodation, the standard must not be betterthan the level You booked for the rest of YourJourney.Relative – means a Spouse, parent, step-parent,parent-in-law, grandparent, child, step-child,grandchild, brother, brother-in-law, sister, sister-in-law,son-in-law, daughter-in-law, uncle, aunt, niece,nephew, first cousin, fiancé, or fiancée, all permanentlyresiding in Australia or New Zealand.Spouse – means a legal or de facto Spouse or apartner who is in a permanent relationship. Robberies or other criminal acts, primarily committedfor personal gain and acts arising primarily from priorpersonal relationships between perpetrator(s) andvictim(s) shall not be considered Terrorism. Terrorismshall also include any act which is verified or recognisedby the (relevant) Government as an act of Terrorism.Travelling Companion – means the person/s Youarranged to travel with before You left Your residencein Australia to commence Your Journey. This personmust be a permanent resident of Australia or NewZealand, and be travelling with You for at least 50% ofYour Journey.War – means war or any warlike activities, includinguse of military force by any sovereign nation toachieve economic, geographic, nationalistic, political,racial, religious or other ends.We, Our, Us means American Home AssuranceCompany A.B.N. 67 007 483 267 trading in Australia asChartis (‘Chartis’).You, Your means the person named as the accountholder of a Platinum account (Insured Person) andincludes Your Spouse and dependent persons under21 years of age and not in full time employment whotravel with you on Your Journey. You, Your, excludesany person aged over 80 years of age. Additional orsupplementary cardholders who are not Your Spouse ordependent children under 21 years of age are notcovered.General Policy ExclusionsWhat We Will Not Pay For Under Any Types Of CoverWe will not pay claims for, or which are directly orindirectly caused by, any of the following:1. War, civil war, invasion, insurrection, revolution,use of military power or usurpation of governmentor military power.2. Losses incurred if You:a. have not met the Activation of Cover criteria onthe “Important Matters” section;b. do not have a ticket returning You to Australiawithin six months of the commencement ofYour Journey;c. were over 80 years of age at the time Your coverwas activated.5. Pre-existing Medical Conditions of any person.This includes You, Your Travelling Companions,Your Relatives, or Your Business Partners. This willnot apply to You if You have applied for and Wehave approved additional cover for Your Preexisting Medical Condition(s). You cannot applyfor cover for Pre-existing Medical Condition(s) ofany person not insured under this policy.6. Pregnancy or childbirth. This does not include anymedical complication which occurs before the endof the 24th week of Your pregnancy. The 24thweek is calculated using Your estimated date ofdelivery given to Us by Your Doctor.7. Your failure to make reasonable efforts to:a. safeguard Your property. This includes failure touse any safe or safety deposit facility madeavailable to You; orb. avoid accidental injury; orc. minimise Your loss; ord. avoid a loss, if there has been a warning givenby the general media of an intended strike, riotor civil commotion, severe weather.8. a.
